Answer questions for Parts A through C by checking either "Yes" or "No". Complete Part D of the checklist when Minimum Criteria Rule categories #8, 12(i) or #15 are used. TIP Project No.: SF-050001 State Project No.: Project Location: SR 1349 (Ruffin St.), Linville, NC Project Description: This project replaces existing bridge #01, composed of 2 spans, each 36' in length, for an overall length of 72'. The existing bridge consists of timber floor on I -beams with reinforced concrete abutments and pier, and is 18' wide out -to -out. The proposed new bridge will be two spans, each consisting of 45' cored slabs for an overall length of 90'. The new bridge width will be 33' out to out. There is a slight increase in the impervious area of the project, however there is no significant increase in the discharge from the roadway runoff. Anticipated Permit or Consultation Requirements: Nationwide Permit #3 from USACE and Water Quality General Certification 4132 from NCDWR Special Project Information: Environmental Commitments Construction Moratorium - Brown Trout occur in the project area. As a result, NCWRC has requested a moratorium prohibiting in -stream work and land disturbance within the 25-foot trout buffer from October 15 to April 15 to protect the egg and fry stages of trout. Federally Protected Species - Suitable habitat for the Northern Long -Eared Bat (NLEB) does exist within the project study area. Furthermore, a review of the NC Natural Heritage Program (NCNHP) records during November 2019 revealed that NLEB have been documented within a mile of the project. Due to the known proximity of NLEB, a survey request was made to NCDOT's Biological Surveys Group. Additional information will be provided from their survey by late spring/early summer 2020. 01/13/20 1 of 4 PART A: MINIMUM CRITERIA Item I to be completed by the Engine e YES 1. If any questions in Part "C" are answered "yes", follow the appropriate permitting procedures prior to beginning project construction.
