Commercial Avenue Stantonburg, NC 27883 Dear Mr. Davis: You have our approval, in accordance with the attached conditions and those listed below, to place fill material in an unspecified length of streams for the purpose of constructing the Speight School Utility Extension, as you described in your application dated May 15, 2000. After reviewing your application, we have decided that this fill is covered by General Water Quality Certification Number 3101. This certification allows you to use Nationwide Permit Number 12 when the Corps of Engineers issues it. This approval is also valid for the Neuse River buffer rules (15A NCAC 2B .033). In addition, you should get any other federal, state or local permits before you go ahead with your project including (but not limited to) Sediment and Erosion Control, Coastal Stormwater, Non-Discharge and Water Supply Watershed regulations. This approval will expire when the accompanying 404 or CAMA permit expires unless otherwise specified in the General Certification. This approval is only valid for the purpose and design that you described in your application except as modified below. If you change your project, you must notify us and you may be required to send us a new application.. If the property is sold, the new owner must be given a copy of this Certification and approval letter and is thereby responsible for complying with all conditions. If total wetland fills for this project (now or in the future) exceed one acre, compensatory mitigation may be required as described in 15A NCAC 2H .0506 (h) (6) and (7). This approval shall expire when the corresponding Nationwide Permit expires or as otherwise provided in the General Certification. For this approval to be valid, you must follow the conditions listed in the attached certification and any additional conditions listed below. I. All stream crossings shall be between 75 and 105 degrees and be equal to or less than 40 feet in width. If you do not accept any of the conditions of this certification, you may ask for an adjudicatory hearing. You must act within 60 days of the date that you receive this letter. To ask for a hearing, send a written petition, which conforms to Chapter 150B of the North Carolina General Statutes to the Office of Administrative Hearings, P.O. Box 27447, Raleigh, N.C. 276 1 1-7447. This certification and its conditions are final and binding unless you ask for a hearing. Before we can proceed with reviewing your application, we need the following information: 1. A vicinity map and site map with the impacted stream clearly identified; 2. A project narrative description, 3. Please note that this project occurs within the Neuse River Basin, which has Buffer Rules in effect. PROJECT NARRATIVE o ®o 5 5 STREAM CROSSING SUMMARY SPEIGHT MIDDLE SCHOOL UTILITY EXTENSIONS TOWN OF STANTONSBURG WILSON COUNTY, NORTH CAROLINA Project Description The purpose of this project is to extend water and sanitary sewer to serve the proposed Speight Middle School Northwest of Stantonsburg, North Carolina. The project will include construction of approximately 6,100 LF of 8" ? Water Main with appurtenances, approximately 11,554 LF 6" ? Force Main and one (1) Pump Station to serve the proposed Speight Middle School and users along Old Stantonsburg Road (SR 1602). The project is located approximately 2.2 miles Northwest of the Town of Stantonsburg, North Carolina and water main and force main will be constructed entirely within the right of way of SR 1628 and SR 1602 and will have a total project disturbed area of approximately 5 acres. All stream crossings will consist of placing pipe beneath any drainage structure to be crossed with a minimum of 18" of vertical clearance. No streambed excavation or disturbance will take place on this project. All streams being crossed are tributary to Contentnea Creek just West of the Town of Stantonsburg. Stream Crossing Locations There are four intermittent stream crossing locations on this project. Stream Crossing #1 The first stream crossing is found on Sheet 1 of 2 Construction Plans and consists of 6" ? force main crossing under an 18" ? RCP at Station 20+36.15 within the Woodbridge Road (SR 1628) Right of Way. A minimum of 18" clearance will be maintained beneath the existing drainage pipe and no streambed disturbance will take place due to the force main being constructed along the shoulder of said roadway. Stream Crossing #2 The second stream crossing is located at Station 45+81.34 on Sheet 1 of 2 Construction Plans and is a continuation of said force main along Woodbridge Road (SR 1628). As before, no streambed disturbance will take place as force main will be constructed beneath existing 48" ? CMP with 18" minimum clearance and within the shoulder of Woodbridge Road. Stream Crossing #3 The third stream crossing is located at Station 82+88.64 on Sheet 2 of 2 Construction Plans. This stream crossing will include a continuation of 6"c force main on East shoulder of Old Stantonsburg Road (SR 1602) and 8" ? water main on West shoulder. Neither stream crossing will disturb existing streambed and 18' clearance will be maintained beneath existing 18"c RCP. Stream Crossing #4 The fourth stream crossing is found at Station 87+10.47 on Sheet 2 of 2 Construction Plans. As with Stream Crossing #3 this stream crossing will include 6" ? force main on East shoulder and 8" ? water main on West shoulder of Old Stantonsburg Road (SR 1602). Once again, no streambed will be disturbed as pipe will be directed under existing 18" ? RCP with a minimum of 18" vertical clearance. PROJECT NARRATIVE STREAM CROSSING SUMMARY SPEIGHT MIDDLE SCHOOL UTILITY EXTENSIONS TOWN OF STANTONSBURG WILSON COUNTY, NORTH CAROLINA Project Description The purpose of this project is to extend water and sanitary sewer to serve the proposed Speight Middle School Northwest of Stantonsburg, North Carolina. The project will include construction of approximately 6,100 LF of 8" ? Water Main with appurtenances, approximately 11,554 LF 6" ? Force Main and one (1) Pump Station to serve the proposed Speight Middle School and users along Old Stantonsburg Road (SR 1602). The project is located approximately 2.2 miles Northwest of the Town of Stantonsburg, North Carolina and water main and force main will be constructed entirely within the right of way of SR 1628 and SR 1602 and will have a total project disturbed area of approximately 5 acres. All stream crossings will consist of placing pipe beneath any drainage structure to be crossed with a minimum of 18" of vertical clearance. No streambed excavation or disturbance will take place on this project. All streams being crossed are tributary to Contentnea Creek just West of the Town of Stantonsburg. Stream Crossin Locations There are four intermittent stream crossing locations on this project. Stream Crossing #1 The first stream crossing is found on Sheet 1 of 2 Construction Plans and consists of 6" ? force main crossing under an 18" ? RCP at Station 20+36.15 within the Woodbridge Road (SR 1628) Right of Way. A minimum of 18" clearance will be maintained beneath the existing drainage pipe and no streambed disturbance will take place due to the force main being constructed along the shoulder of said roadway. Stream Crossing #2 The second stream crossing is located at Station 45+81.34 on Sheet 1 of 2 Construction Plans and is a continuation of said force main along Woodbridge Road (SR 1628). As before, no streambed disturbance will take place as force main will be constructed beneath existing 48" ? CMP with 18" minimum clearance and within the shoulder of Woodbridge Road. Stream Crossing #3 The third stream crossing is located at Station 82+88.64 on Sheet 2 of 2 Construction Plans. This stream crossing will include a continuation of 6" ? force main on East shoulder of Old Stantonsburg Road (SR 1602) and 8" ? water main on West shoulder. Neither stream crossing will disturb existing streambed and 18' clearance will be maintained beneath existing 18" ? RCP. Stream Crossing #4 The fourth stream crossing is found at Station 87+10.47 on Sheet 2 of 2 Construction Plans. As with Stream Crossing #3 this stream crossing will include 6" ? force main on East shoulder and 8" ? water main on West shoulder of Old Stantonsburg Road (SR 1602). Once again, no streambed will be disturbed as pipe will be directed under existing 18" ?
