Dear Mr. Whaley:
You have our approval, in accordance with the attached conditions, to impact 0.5 acres of wetlands in Onslow County
for the'purpose of sewer and water main construction, as you described in your application received by the Division of
Water Quality on April 27, 2000. After reviewing your application, we have determined that this action is covered by
General Water Quality Certification Number 3101. This Certification allows you to use Nationwide Permit Number 12
when issued by the U.S. Army Corps of Engineers. In addition, you should get any other federal, state or local permits
before you go ahead with your project including (but not limited to) Sediment and Erosion Control, Non-Discharge and
Water Supply Watershed regulations. Also this approval will expire when the accompanying 404 permit expires unless
otherwise specified in the General Certification.
This approval is only valid for the purpose and design that you described in your application except as modified below. If
you change your project, you must notify us and you may be required to send us a new application. If the property is sold, the
new owner must be given a copy of this Certification and approval letter and is thereby responsible for complying with all
conditions. If total wetland fills for this project (now or in the future) exceed one acre, compensatory mitigation may be required
as described in 15A NCAC 2H .0506 (h) (6) and (7); This approval shall expire when the corresponding Nationwide Permit
expires or as otherwise provided in the General Certification. For this approval to be valid, you must follow the conditions listed
in the attached certification.
If you do not accept any of the conditions of this certification, you may ask for an adjudicatory hearing. You must act
within 60 days of the date that you receive this letter. To ask for a hearing, send a written petition, which conforms to Chapter
150B of the North Carolina General Statutes to the Office of Administrative Hearings, P.O. Box 27447, Raleigh, N.C. 27611-
7447. This certification and its conditions are final and binding unless you ask for a hearing.
This letter completes the review of the Division of Water Quality under Section 401 of the Clean Water Act. If you have

Improvements to Town sanitary sewer and water distribution systems include laving sewer and water mains
under creek and stream beds in four (4) locations with excavating equipment (backhoes)

PROJECT NARRATIVE
STREAM CROSSINGS DESCRIPTIONS
TRI-FIELD ESTATES - PHASE I
RICHLANDS, NORTH CAROLINA
Project Description
The purpose of this project is to construct Phase One of Tri-Field Estates subdivision.
This project will include the construction of approximately 2300 LF of curb and guttered
roadway, approximately 3,350 LF of 8" ? sanitary sewer, approximately 1,840 LF of 4" ? force
main and approximately 2,200 LF of 8" ?. and 6" ? waterline. A total of 7.80 acres will be
disturbed with the construction of this project. The site is approximately 20 acres located in
Onslow County east of the Town of Richlands.
Stream Crossings
There are four blue line stream crossings on this project.
Stream Crossing # 1
The first of these crossings is shown on sheet 1 of 1 of the construction plans. This
sheet contains an aerial view of the water main crossing the Squires Run Stream. This 8" ?
stream crossing will be constructed at 90° crossing the stream and will have adequate cover
beneath the streambed. This stream crossing is to be constructed entirely in the right of way of
Francktown Road (SR 1307)
Stream Crossing # 2
The second stream crossing is located in the proposed Tri Field subdivision at Station
6+63.74 on Street "A". The profile of this crossing is shown on sheet 1 of 12 of the construction
plans. This crossing of an unnamed tributary of Squires Run Stream consists of an 8" ? water
line and sewer line both crossing the stream at 90° relative to the stream and is being installed
with a minimum of 3 feet of cover beneath the streambed.
Stream Crossing # 3
The third stream crossing is located at approximately Station 8+50 of the Off - Site
Sewer line and is shown in profile on sheet 11 of 12 of the construction plans. This crossing of
an unnamed tributary of Squires Run Stream consists of a 6" ? force main installed at 90°
relative to the stream with a minimum of 3 feet of cover beneath the streambed.
Stream Crossing # 4
The final stream crossing associated with this project is located from Station 11+50 -
Station 15+00 on the Off Site Sewer line and is shown in profile on sheet 11 of 12 of the
construction plans. This proposed crossing of an unnamed tributary to Squires Run Stream is to
be installed at 90° relative to the stream and beneath the streambed with at least 4 feet of cover as
shown in the construction plans. This crossing will be installed by a 400 LF directional bore and
will consist of 8" ? HDPE pipe.
