Describe the attachments:
As observed at the site visit on 1/30/2020, the authorized culvert location at Site 4 does not line up with the
actual stream location on the site. NCDOT requests authorization to relocate the 42" RCP to the new location
depicted on the attached revised permit drawing to convey the streamflowthrough the site. Culvert will be
buried 0.7% per the attached drawing. Due to construction having started at the site, expedience approving
this request would be greatly appreciated.
