As requested, I am submitting this letter to recap the issue of our returned DMR submitted the first of December 2019. I reached out to our Washington Regional office and spoke with Thom Edgerton. I explained that per the tracking, our certified mailed DMR had been delivered on December 5th and was return to us over the holidays with the USPS yellow stamp of "UNABLE TO FORWARD/FOR REVIEW". Mr. Edgerton advised me to contact your office as this may be an opportunity that affects other individuals. I have included the returned DMR with this letter.
