Dear Mr. Wrenn:
The North Carolina Department of Transportation (NCDOT) proposes to replace a non-
inventory metal deck bridge located on SR 1654 (Meurer Terrace Road) on the Pfieffer
University campus located in Misenheimer. The purpose of the proposed project is to
address load bearing issues associated with the existing structure. The current bridge
cannot be rehabilitated.
The NCDOT Hydraulics Unit has recommended that the current structure be replaced
with a three sided concrete box culvert (open bottom). The NCDOT proposes to install a
pre-constructed three sided box culvert over Curl Tail Creek that measures 35 feet wide
by 40 feet long. The structure will span Curl Tail Creek, however, NCDOT would like to
request permission to stabilize the creek banks with Class H rip rap to prevent the
possibility of bank loss due to the lack of vegetation. Bank stabilization activities would
require 40 linear feet of rip rap and will be limited to the area from the toe of slope to the
top of the bank. As previously referenced, the construction of this project would impact
Curl Tail Creek which is located in the Yadkin River Basin, Hydrologic Unit 03040105.
The NCDOT anticipates that this project will be authorized under a Section 404
Nationwide Permit No. 14 and we are requesting authorization from the North Carolina
Division of Water Quality to utilize the corresponding 401 Water Quality Certification
GC3704 based upon the enclosed information.

3. Project Description
3a. Describe the existing conditions on the site and the general land use in the vicinity of the project at the time of this
.application:
SR 1654 (Meurer Terrace Road) is a paved road located on the campus of Pfeiffer University that has been recently
added to the state road system. The property has been developed with adminstrative buildings, parking lots, an athletic
facility and sturdent housing. The majority of the site has been impacted at one time or another with the exception of a
sparsely vegetated stream buffer along Cud Tail Creek.

This is a linear transportation project and the stream crosses parallel to the roadway. The stream extends beyond the
NCDOT 50-foot right-of-way.
3d. Explain the purpose of the proposed project:
The purpose of this project is to improve the safety of the roadway by replacing a non-inventory metal bridge that is not
currently posted. The bridge is used by school buses and does not meet the minimum load bearing requirements to carry
overweight vehicles. NCDOT proposes to replace the existing bridge with a 3 sided box culvert that measures 35-foot
wide by 40 feet in length.
3e. Describe the overall project in detail, including the type of equipment to be used:
Traffic will be routed on-site to a temporary bridge that spans Curl Tail Creek. The temporary bridge will be located
upstream of the bridge that is to be replaced. The existing bridge will be removed using DOTS best management
practices for bridge removal and construction will begin on the foundation for the three sided box culvert. It is anticipated
that the foundation can be constructed without impact to the stream. The Department my opt to use lined "jersey
barriers" at the edge of the stream bank to keep water from inundating the foundation during construction. The three
sided box culvert will be preconstructed off-site and installed in sections with the use of a crane. Once the culvert
construction has been completed, the wing walls will be backfilled, the roadway paved, and the temporary bridge
removed. Typical construction equipment will be used: trackhoe, crane, dump trucks, and paving equipment. Erosion
and sediment control measures will be installed prior to beginning any work.

1. Avoidance and Minimization
1a . Specifically describe measures taken to avoid or minimize the proposed impacts in designing project.
The proposed impacts for this project have been limited to those necessary for the removal of the existing bridge and the
construction of the new 3 sided box culvert. The existing structure has stone headwalls located along the stream banks
while the proposed structure will span the creek, creating more flood prone area. Measures will be taken to prevent any
additional impacts to the stream by adhereing to NCDOT's BMP's.

The foundation for the replacement structure can be constructed outside of the stream and the equipment required to
construct the foundation will be able to access both sides of the creek by way of the temporary bridge. Curl Tail Creek
will not need to be dewatered during construction but silt fence and barrier fencing will be used to delineate the
construction site and prevent soil loss to the stream during rain events. NCDOT is proposing to add Class II rip rap along
the footers on both sides of the creek to prevent bank loss due to lack of vegetation.
