GENERAL INFORMATION:
GENERAL PROGRAM INFORMATION:
• The Headworks Analysis was revised for Dissolved Metals standards and was received on
8/16/19. An email was received from Monti Hassan on 11/4/19 stating agreement with the
calculations.
• A revised LTMP was submitted on 12/4/19.
• A revised ERP to address 40 CFR Part 441 Requirements was submitted on 1/4/2020.
• All other dates and information on the Division's Pretreatment Program Information Sheet are
correct.
• A copy of our Program Info sheet is enclosed.
GENERAL PERMIT INFORMATION:
• No permits lapsed or expired prior to renewal
SIU INFORMATION:
Mars Petcare(IUP#0011,Pet Foods)
SNC and other INFORMATION:
SRI was not in SNC this PAR Year.
Please note on the IDSF form that: Arsenic,Cadmium,Chromium,Lead,Mercury,Nickel, Selenium,
Silver,Molybdenum and Ammonia were listed as not required for the 1st 6 month period because our
LTMP annual monitoring requirements for these parameters were met in the 2nd 6 month period.
