Inspection Summary:
Dear Mr. and Mrs. Elmore:
Staff of the Mooresville Regional Office (MRO) conducted a compliance inspection on September 25th, 2019. The MRO
would like to thank your AQWA technicians (Chad and Jeff) for their time during the site visit. The treatment and irrigation
systems appeared to be operating adequately at the time of inspection. Please note the following comments to maintain
overall program compliance:
* The permit requires connection to a municipal system should access become available in your area; otherwise, a renewal
application is due 6 months prior to the expiration date of September 30th, 2020.
For future reference a renewal form may be located at the NCDEQ website below.

* The application field needs typical maintenance for underbrush, downed trees and wire fence repair (side facing the house)
Only one typical dripline repair was needed and corrected by your contractors at the time of inspection. The MRO is aware
that wooded application sites require consistent maintenance of driplines, underbrush and fencing.
* Per the previous inspection report, you were to maintain documentation of the required owner weekly UV and monthly
system/field inspections. Please email a copy (or photo) of your most recent page of entries to my address below.

Comment: MRO was notified of AQWA planned visit last week and met technicians, but did not request
owner presence.
Septic Tank
*`* The septic tank and filters should be checked annually and pumped/cleaned as needed.

AQWA techs assessed levels via sludge judge are adequate and AQWA does not recommend pumping at
this time. In main septic tank scum layer was at 2" and sludge thickness at 12". All remaining tank sludge
layers were much less.

If FILTER, when was the filter cleaned and by who?
Comment: Maintenance contract is for two visits per year (approx. every 6 months), unless an alarm
response indicates required repair.
Sand Filter/Treatment Pods

Comment: AQWA staff cleaned/replaced UV bulbs while onsite. Owner is requested to email copy of
weekly/monthly checks. System is connected to telemetry and call when system indicates an
issue.

Comment: Permittee presence was not request at this inspection, but per previous conversations the
permittee is familiar with the system (Is also a plumber by trade). Per AQWA tech, the high
water alarm is on a 24hr delay to allow for unusual (extra company/party, etc.) one day flow to
self correct.

Comment: The standard two -strand wire fence is in place. There is only 1 wire on the side nearest the
home. The forested application area needs routine manitenance for underbrush (poison ivy),
downed trees and repair of the wire fence facing the home (brush/tree across wire). Only one
dripline repair in zone 2 was needed and completed by AQWA technicians during this site visit.
