On January 7, 2020 staff of the NC Division of Water Resources (DWR), Water Quality Regional
Operations Section, inspected the subject wastewater treatment and disposal system. The purpose
of the inspection was to conduct a compliance inspection.
The following items specify permit conditions and limitations that were in violation during the
time of the inspection. (Also, please refer to the attached inspection report):
OPERATION AND MAINTENANCIE RE UIRENIENTS:
Violation # 1, Condition I., N%-hich states, `'The facilities shall be properly maintained and operated at all

the proper operation of the spray heads, that the system is free of leaks, that the vegetative gron-th allo«'s

In addition to the above deficiency's, i have included a copy of NOV-2019-PC-0379 which was issued
May 10, 2019 for overdue annual fees. Please be advised that VI. GENERAL CONDITIONS 10., which
states "The Permittee shall pay the annual fee within 30 days after being billed by the Division. Failure to
pay the annual fee accordingly shall be cause for the Division to revoke this permit" At the time of this
letter annual fees for 2017, 2018, 2019 are overdue and 2020 is due on or before February 9, 2020. Pleawe
make arrangements as soon as possible to satisfy this permit requirement.
To prevent further action, please respond in ►rriting to this office Niithin 3.10 days of receipt of this
letter, please include your plans or measures to be taken to address the indicated violations and other
identified issues, if applicable. Please include in your response all corrective actions already taken and a
schedule for completion of any corrective actions not addressed. Please inform our office if there are

that we may be of assistance to help you bring this system into compliance. Failure to comply with the

If the non -discharge system has not been constructed, or if it is no longer active, the permit may be
rescinded. By rescinding the permit, all overdue annual fees will be waived; however, if it is desired to
again construct and/or operate the subject non -discharge system, a new non -discharge permit shall be
applied for and received. Permit rescission requests may be emailed to Non-

If the Permittee no longer owns the non -discharge system, the Permittee is still res onsible for the
permit and annual -fee . As noted in the permit, the subject permit is not transferrable with the land. A
Change of Ownership application shall be submitted, and a permit reissued to the new landowner to release
the current Permittee from all responsibilities. For the most recent Change of Ownership application, please

completed by both the current Permittee and the new landowner, and mailed to the address listed on the
