Narrative
CITY OF HICKORY 2019 PRETREATMENT ANNUAL REPORT
The Annual Pretreatment Report submitted by the City of Hickory contains
information from January 1, 2019 — December 31, 2019. In this period of time,
13 industries were monitored.
JANUARY - JUNE 2019
• POTW monitoring for thirteen (13) industries occurred. Thirty-two
(32) monitoring events were conducted by the City of Hickory.
• Self -monitoring by thirteen (13) industries occurred. Seventy-six
(76) self -monitoring events were conducted.
• Zero (0) industries were in SNC for limits or reporting.
• Eleven (11) industries were sent thirty-three (33) [self -monitoring
data — sixteen (16); City of Hickory data — seventeen (17)] notices
of noncompliance with the Industrial Monitoring Program.
• During February 2019, one (1) industry was sent one (1) notice of
violation with assessment of civil penalties for violation of permit
conditions (Part 2 Section 24 — Dilution Prohibition) due to a
violation that occurred on 1/23/19. The civil penalty was assessed
and collected in 2019. (See Other Information)
Kleen Tech (IUP# 0035) conducted TTO monitoring. (See
enclosures)
The following IUP modifications were made during this semi-annual
period:
Kleen Tech (IUP# 0035) — BOD and O&G limits increased.
• The following Northeast (NC0020401) IUP's were submitted for
renewal with approval on 2/26/19:
1, Frye Regional Medical Center, Pipe 001 & 002 (IUP# 0007)
Hickory Dyeing &Winding (IUP# 0008)
*Reduction in O&G limit*
3. HK Research (IUP# 0011)
4. Shurtape Technologies (IUP# 0022)

POTW monitoring for thirteen (13) industries occurred. Fifty-five
(55) monitoring events were conducted by the City of Hickory.
• Self -monitoring by thirteen (13) industries occurred. Seventy-three
(73) monitoring events occurred.
• Zero (0) industries were in SNC for limits or reporting.
Ten (10) industries were sent forty-two (42) [self -monitoring data —
twenty (20); City of Hickory data — twenty-two (22)] notices of
noncompliance with the Industrial Monitoring Program.
• The City of Hickory conducted TTO monitoring for Kleen Tech
(IUP# 0035). (See Enclosures)
• The following IUP renewals became effective on 7/8/19:
1. Frye Regional Medical Center, Pipe 001 & 002 (IUP# 0007)
2. Hickory Dyeing & Winding (IUP# 0008)
3. HK Research (IUP# 0011)
4. Shurtape Technologies (IUP# 0022)
5. Century Furniture, Pipe 001 & 002 (IUP# 0034)
Other Information:
A copy of the Division's Historical SNC Report has been included.
Please note the following correction that is needed:
Kleen Tech IUP# 0035 was in SNC for Oil and Grease
during the 2nd semi-annual period of 2017 and isn't listed on
the Historical SNC.
• A copy of the Division's Pretreatment Program Database has been
included for each WWTF with an active PT program. No
corrections are needed.
IDSF data listed as "not required" for asemi-annual period
indicates that the corresponding industry does not have permit
requirements for said parameter and that the monitoring for the
other semi-annual period was conducted to satisfy LTMP and/or
MMP requirements only.

Pretreatment Program Element Updates:
• Henry Fork (NC0040797) HWA was updated and approved on 7/18/19.
• Henry Fork (NC0040797) LTMP was updated and approved on 7/18/19.
• ERP was updated to comply with the Dental Amalgam Rule and approved
on 11/13/19.
• The City of Hickory has implemented the Dental Amalgam Rule by
identifying all dental dischargers in the Hickory area. Survey efforts began
on 7/16/18 by sending packets that included the model cover letter
explaining the Dental Amalgam Rule, EPA fact sheet, and One -Time
Compliance Report to 37 dental dischargers. A summary table is being
used to indicate the dates that the One -Time Compliance Reports are
being received.
Henry Fork (NC0040797) IU Permit Renewals:
• Permit renewal applications for City of Hickory c/o Veolia Water North
America Operating Services (IUP# 0050) and Maple Springs Laundry —
Plant 2 (IUP# 0051) will be sent to SIU's in 212020 and 5/2020
respectively.
• Permit renewal applications for the remainder of Henry Fork SIU's will be
sent to SIU's in 7/2020.

This letter is in response to the noncompliance letter dated February 06,
2019 for Assessment to Civil Penalty.
The issue was corrected on January 28, 2019 at 14:30.
Maintenance was instructed about issue with the hose, the non-compliance
and also informed about Part 2 Section 24. This issue has been resolved.
