1. Reviewer Comments: Documentation from the NC Secretary of State or other official documentation
showing the title and position held by the person who signed the application is required [15A NCAC
02H .1042(b)(ii)]. This can also be in the form of a letter signed by the registered agent or a principal
executive office of the level of vice-president or higher [15A NCAC 02H .1040(1)] granting authority to
the individual who signed the application.
Response: Attached is the requested letter from Lumbee River EMC.
2. Reviewer Comments: Please complete Part III.4 of the SWU-101 application with the contact
information for the local jurisdiction for building permits.
Response: The information has been added to the application as requested.
3. Reviewer Comments: Please complete a low density supplemental form. The Supplement EZ form
can be downloaded from the Stormwater Design Manual website.
Response: Attached is the requested form.
4. Reviewer Comments: Please place the swale in a permanent recorded easement that extends to the
nearest public right-of-way in accordance with General MDC 9. The substation does not need to be
included in the easement.
Response: An easement has been added to the plans as requested. See revised sheets.
