Inspection Summary:
Site visit was to follow up on an emergency situation that occurred over the weekend (May 28-29). Two buildings that
housed chemicals caught fire; appropriate personal responded during the emergency situation.
Fire suppression waters discharged to small basin (S.E.) and via overland flow which reconcentrated (N.) to small gully
under fence. EPA overseeing cleanup, small basin contents were being pumped to container storage for disposal. Fire
suppression waters which left the sight via overland flow to receiving stream were not reported as causing fish kill or
apparent violations.
DWQ told facility consultant(Inenco)and Director (Davis Phillips) to gather stormwater samples during next rain/discharge
event to determine if contaminants remain, and to include in this sampling event Organics analysis. Then to resume
NCG09 stormwater monitoring as required, but to include all SDO's.

Comment: Facility indicated it has performed analytical monitoring at one SDO in the
past. Three SDO's were noted during site visit. Facility was told to update SP3 to
indicate all SDOs. The middle SDO is an overflow situation that reconcentrates at a
small ditch under fence and is the most likely SDO to be a concern to receiving waters.

Highlights from EM-43 reports.
5/28/11 —
1624 - 1 mile radius evacuation.
1715 - Charlotte Fire advised they were responding with a foam unit, tanker, Battalion Chief, and Field Communications and 12 person
RRT. FDs on scene from: Caldwell (every dept in county), Watauga, Burke, Catawba, Lincoln, Alexander counties.
1842 - J Cardwell (DEM) adv update: Fire under control, hazmat not. RRT6 performing water and air monitoring. STAT Environmental
on scene for cleanup. Ken Rhame has approx 1 hr eta, air and water monitoring equip from Atlanta has approx 4 hr eta.
No injuries. 1 bldg destroyed, 2 buildings with no damage. Main concern is run-off, cleanup company is on scene remediating.
05/29/2011
0045 All units have cleared except 2 FDs and STAT Inc.
0730 - J Cardwell advised that he is back on scene meeting with EPA and other officials.
1115 - J. Cardwell called with update: Plant personnel, STAT Inc. (Cleanup Company), and EPA are on site and projecting that the
plant should be operational by tomorrow, 2nd shift. EPA is overseeing cleanup. The cleanup company has the entire area dammed and
diked, and expects to hand the entire site over to plant personnel tomorrow afternoon. No issues are pending.
2028 - Jeff Cardwell provided final update from evening briefing:
Cleanup continues per US EPA standards, EPA on scene providing oversight
Water monitoring performed both up- and downstream with no significant readings; no fish kill noted. Both living fish and
other aquatic life were observed.
STAT Environmental remaining on scene through tomorrow (Monday)
EPA OSC will be clearing scene tonight (Sunday)
The company plans to return to normal operations for Second Shift tomorrow (Monday)
NC DENR arrives Tuesday for final inspections
