Subject: Application for Section 404 Nationwide Permits 13, 23, 33, and Section 401 Water
Quality Certifications for the replacement of Bridge #43 over Deep Creek on SR 1132
in Carteret County. State Project No. 8.2161301. Federal Aid Project Number BRZ-
1133(5). Debit $240 from WBS 33707.1.1.TIP No. B-4454.
Please find enclosed the PCN form, permit drawings, half-size plan sheets, utility drawings, and
for the above referenced project. A Categorical Exclusion (CE) was completed for this project
on December 18, 2006, and distributed shortly thereafter. Additional copies are available upon
request. The North Carolina Department of Transportation (NCDOT), Division of Highways, in
consultation with the Federal Highway Administration (FHWA), proposes to replace Bridge No.
43 in Carteret County.
Please note that this project is an accelerated bridge project on NCDOT's Maintenance of Effort
list. The NCDOT Administration has deemed these projects highest priority. This project calls
for a letting date of March 16, 2010 and a review date of January 26, 2010; however, the let date
may advance as additional funding becomes available.
Regulatory Approvals
Section 404 Permit: All aspects of this project are being processed by the Federal Highway
Administration as a "Categorical Exclusion" in accordance with 23 CFR 771.115(b). The
NCDOT requests that a Nationwide Permit 23 authorize these activities. We are also requesting
the issuance of a Nationwide Permit 33 for the temporary fill due to culvert installation and a
Nationwide Permit 13 for bank stabilization. (72 CFR; 11092-11198, March 12, 2007).
Section 401 Permit: We anticipate 401 General Certification numbers 3701, 3688, and 3689 will
apply to this project. NCDOT will adhere to all standard conditions of these certifications is

NCDOT Bridge Maintenance Unit records indicate Bridge No. 43 has a sufficiency rating of 46.6 out of a possible 100 for
a new structure.- The bridge is considered functionally obsolete due to a deck geometry appraisal of 2 out of 9.

The project involves replacing a 61-foot bridge with 3 (11' x 8') RCBC's on the existing alignment with an off-site detour.
Standard road building equipment, such as trucks, dozers, and cranes will be used.

The proposed culvert will be at approximately the same grade and location as the existing structure. An off-site detour
will be used. 3:1 fill slopes are designed where practicable.

Project Description
This project consists of approach roadway work and the replacement of Bridge #43 over Deep Creek on SR
1133. The existing 2 lane facility has 8' lanes. The proposed facility will improve to 10' lanes and 3'
shoulders. The existing 2 @ 30.5' (61' total) bridge will be replaced with a 3 @ I P x 8' Box Culvert w/ Sill
in one barrel. There is a wetland in the SW quadrant of the project.
Project Involvement
The project will require widening of the existing road/fill with wider lanes and the addition of wider
shoulders and guardrail. The project will also require raising the grade approximately P at culvert to provide
adequate cover. The drainage at the site consists of existing ephemeral drainage ditches in all four quadrants.
Deep Creek is classified as C. The box culvert will reduce clearance compared to the bridge by 1.2'.
However, considering site hydraulics, the downstream structure (2 @ 142" x 91" Structural Plate Pipe Arch)
and costs the box culvert was determined to be a more practical replacement structure versus a bridge.
Best Management Practices
Best Management Practices (BMPs) utilized on the project are as follows:
-Promotion of sheet flow and infiltration with grassed shoulders and grassed swales.
-No direct discharge into wetlands/buffer.
-Grade of road reduced to extent practical to reduce wetland/stream impacts.
