Oyster Landing Subdivision—Phase IV(Lots 60-73)
Dear Ms. Beaman:
This letter is in reference to your permit application received April 26, 2019 for the construction and
operation of the subject sewer extension. In an email sent July 25, 2019 and December 19, 2019, by Christyn
Fertenbaugh, P.E., your engineer was asked to provide additional information (copy attached) in accordance
with 15A NCAC 02T.0107(e).As of today,this information has not been received,therefore the Division must
return your application as incomplete in accordance with 15A NCAC 02T.0107(e)(2).
Please be advised that construction and/or operation of wastewater collection,treatment,and/or disposal
facilities without a valid permit is a violation of North Carolina General Statute §143-215.1 and may subject
the owner/operator to appropriate enforcement actions in accordance with North Carolina General Statute
§143-215.6A-6C. Civil penalties of up to$25,000 per day per violation may be assessed for failure to secure a
permit required by North Carolina General Statute§143-215.1.
When you have obtained the requested information that is needed to make your application package
complete,you may submit a new application package with the appropriate permit processing fee. If you have

Hello,
As I am doing my end-of-year file cleanup, I have noted that my records indicate that I have not yet received a response
for the additional information request as sent on July 25, 2019 (copied below),for the subject permit application for the
Oyster Landing Subdivision Phase IV LPSS sewer extension located in Onslow County. Please provide a complete
response to the comments below by Friday,January 10,2020. Failure to respond will result in the application being
returned as incomplete and the documents recycled, per 15A NCAC 02T.0107(e)(2). If you have decided to not pursue
this permit application, please let me know and I will withdraw the application.
Please be advised that operating a non-discharge system (including sewer extensions)without a valid Wastewater
Collection Permit may subject the responsible party to a civil penalty of up to$10,000 per day.

Hello,
A review of the application and supporting documents for the subject application has been completed by this office. In
accordance with 15A NCAC 02T.0107, please provide the additional or revised information for the following comments:
General Review:
1. Please provide a set of revised,final plans and supplementary documentation on digital media (CD or other
digital format)for our records.
2. Please clarify if the flow for these 14 homes was previously permitted under the gravity sewer permit
(WQ0034762)or if flow should be listed in this permit.
3. On the provided Developer's Operational Agreement, Item#5 on the first page is incomplete(the name of the
Unit Owner's Association to be formed). Please revise.
Application:
1. Please provide confirmation that all residences are to be 2-bedroom homes, as the requested flows are 240 GPD
per home. For future submittals,this should be specified on the application in Section VI(8).
Calculations:
1. Please provide downstream capacity calculations for the receiving sewer, per 15A NCAC 02T.0305(b)(2).
2. In the provided calculations, based on the on/off/alarm levels of the pump station as well as the capacity per
vertical foot,the pump station would need to be approximately 6.13 feet in height to accommodate 24-hours of
storage(240 gallons) above the pump-on position, per 15A NCAC 02T.0305(h)(1)(D). Please clarify and revise as
necessary,as the pump station is indicated to be only 6 feet in height.
Plans/Specifications:
1. On the provided plan sheet,the plan view of the low pressure force main appears to depict only 330 linear feet
of 2-inch force main,whereas the model and the application list 460 linear feet as being the length to be
permitted. Please clarify and revise as necessary.
2. Please revise to clearly show a cleanout at the ends of the proposed extension for the purpose of
clearing/flushing the line, per the Minimum Design Criteria.
3. The provided buoyancy calculations indicate that 0.7 cubic yards (-18.9 cubic feet)of concrete are required for
anti-flotation, however,the provided plans indicate that only 16 cubic feet shall be provided. Please clarify and
revise as necessary, per 15A NCAC 02T.0305(b)(2)and the Minimum Design Criteria.
A response to the above comments is considered required in order to proceed with the review of the application and
issuance of the permit. If a complete response is not received within 30 days,the application and supporting documents
will be returned to you in accordance with 15A NCACO2T.0107(e)(2).
