TROGDON, III SECRETARY A pre -construction meeting was held on December 18, 2019 for the above referenced project. DESCRIPTION: REPLACEMENT OF BRIDGE #35 OVER BRANCH OF SLADE CREEK Project Schedule/Contractor Schedule: Date of Availability: November 18, 2019 Completion Date: May 15, 2020 Contractor's Plan_ of Operation Start Date: January 13, 2020 A revised start date of January 21, 2020 has been submitted by contractor after precon Discussion/Plans: Conditions that were discussed Condition 6: Suitable Material No activity may use unsuitable material (e.g., trash, debris, car bodies, asphalt, etc.). Material used for construction or discharged must be free from toxic pollutants in toxic amounts (see section 307 of the Clean Water Act). For any fill we will use Sawyer's commercial source. Condition 11: Equipment: Heavy equipment working in wetlands or mudflats must be placed on mats, or other measures must be taken to minimize soil disturbance. Condition 21: Discovery of Previously Unknown Remains and Artifacts. If you discover any previously unknown historic, cultural or archeological remains and artifacts while accomplishing the activity authorized by this permit, you must immediately notify the district engineer of what you have found, and to the maximum extent practicable, avoid construction activities that may affect the remains and artifacts until the required coordination has been completed. The district engineer will initiate the Federal, Tribal, and state coordination required to determine if the items or remains warrant a recovery effort or if the site is eligible for listing in the National Register of Historic Places. We need to stop and see if we need to do any surveys. Condition 2: The Permittee shall ensure that the final design drawings adhere to the permit and to the permit drawings submitted for approval. [15A NCAC 02H .0507(c) and 15A NCAC 02H .0506 (b)(2) and (c)(2)]. Drawings that was submitted has been revised on July 9, 2019. Condition 3: The outside buffer, wetland or water boundary located within the construction corridor approved by this authorization shall be clearly marked by highly visible fencing prior to any land disturbing activities. Impacts to areas within the fencing are prohibited unless otherwise authorized by this certification. [15A NCAC 02H.0501 and .0502]. We will use orange fence and marked flags. Condition 4: During the construction of the project, no staging of equipment of any kind is permitted in waters of the U.S. or protected riparian buffers. [15A NCAC 02H.0506(b)(2)]. TROGDON, III SECRETARY Condition 5: All mechanized equipment operated near surface waters must be regularly inspected and maintained to prevent contamination of stream waters from fuels, lubricants, hydraulic fluids, or other toxic materials. [15A NCAC 02H.0506(b)(3)] Condition 8: When applicable, all construction activities shall be performed and maintained in full compliance with G.S. Chapter 113A Article 4 (Sediment and Pollution Control Act of 1973). Regardless of applicability of the Sediment and Pollution Control Act, all projects shall incorporate appropriate Best Management Practices for the control of sediment and erosion so that no violations of state water quality standards, statutes, or rules occur. [15A NCAC 02H .0506{b)(3) and (c)(3) and 15A NCAC 02B .0200 Condition 9: Sediment and erosion control measures shall not be placed in wetlands or surface waters or within 5 feet of the top of bank without prior approval from DWR. [15A NCAC 02H.0506(b)(3) and (c)(3)]. We will use Erosion & Control matting, using Coir Fiber Mat or Natural Fiber Mat. Condition 11: If placement of sediment and erosion control devices in wetlands and waters is unavoidable, then design and placement of temporary erosion control measures shall not be conducted in a manner that may result in disequilibrium of wetlands, stream beds, or banks, adjacent to or upstream and downstream of the above structures. All sediment and erosion control devices shall be removed from wetlands and waters and the natural grade restored within two (2) months of the date that the Division of Energy, Mining and Land Resources (DEMLR) or locally delegated program has released the specific area within the project. [15A NCAC 02H.0506(b)(3) and (c)(3)] Condition 12: As a condition of this 401 Water Quality Certification, the bridge demolition and construction must be accomplished in strict compliance with the most recent version of NCDOT's Best Management Practices for Construction and Maintenance Activities. [15A NCAC 02H .0507(d)(2) and 15A NCAC 02H .0506(b)(5)] Condition 13: Bridge deck drains shall not discharge directly into the stream to the maximum extent practicable. Stormwater shall be directed across the bridge and pre-treated through site -appropriate means (grassed swales, pre- formed scour holes, vegetated buffers, etc.) where possible before entering the stream. To meet the requirements of NCDOT's NPDES permit NCS0000250, please refer to the most recent version of the North Carolina Department of Transportation Stormwater Best Management Practices Toolbox manual for approved measures. [15A NCAC 02H .0507(d)(2) and 15A NCAC 02H .0506(b)(5)] Condition 14: All bridge construction shall be performed from the existing bridge, temporary work bridges, temporary causeways, or floating or sunken barges. If work conditions require barges, they shall be floated into position and then sunk. The barges shall not be sunk and then dragged into position. Under no circumstances should barges be dragged along the bottom of the surface water. [15A NCAC 02H .0506(b)(3)] Condition 15: Bridge piles and bents shall be constructed using driven piles (hammer or vibratory) or drilled shaft construction methods. More specifically, jetting or other methods of pile driving are prohibited without prior written approval from the NCDWR first. [15A NCAC 02H.0506(b)(2)] Condition 17: No drill slurry or water that has been in contact with uncured concrete shall be allowed to enter surface waters. This water shall be captured, treated, and disposed of properly. [15A NCAC 02H .0506(b)(3) We will also evaluate tide. Condition IS: A turbidity curtain will be installed in the stream if driving or drilling activities occur within the stream channel, on the stream bank, or within 5 feet of the top of bank, or during the removal of bents from an old bridge. This condition can be waived with prior approval from the NCDWR. [15A NCAC 02H.0506(b)(3) STATE OF NORTH CAROLINA DEPARTMENT OF TRANSPORTATION Roy COOPER GOVERNOR JAMES H. TROGDON, III SECRETARY Condition 19: Pursuant to 15A NCAC 2B.0259(6), sediment and erosion control devices shall not be placed in Zone 1 of any Tar -Pamlico Buffer without prior approval by the NCDWR. At this time, the NCDWR has approved no sediment and erosion control devices in Zone 1, outside of the approved project impacts, anywhere on this project. Moreover, sediment and erosion control devices shall be allowed in Zone 2 of the buffers provided that Zone 1 is not compromised, and that discharge is released as diffuse flow. Condition 20: All riparian buffers impacted by the placement of temporary fill or clearing activities shall be restored to the preconstruction contours and revegetated. Maintained buffers shall be permanently revegetated with nonwoody species by the end of the growing season following completion of construction. For the purpose of this condition, maintained buffer areas are defined as areas within the transportation corridor that will be subject to regular NCDOT maintenance activities including mowing. The area with non -maintained buffers shall be permanently revegetated with native woody species before the next growing season following completion of construction. (15A NCAC 213.0259). Condition 21: All stormwater runoff shall be directed as sheet flow through stream buffers at non -erosive velocities, unless otherwise approved by this certification. (15A NCAC 2B.0259) Condition 22: New roadside ditches that do not control for nitrogen and attenuate flow before discharging through the riparian buffer are prohibited [15A NCAC 2B.0259(6)]. Condition 23: NCDOT shall be in compliance with the NCS00250 issued to the NCDOT, including the applicable requirements of the NCGO1000 there will be a drop box with inspection forms. Seeding has a 7-day time frame and will be using Sadler. Condition 26: The permittee and its authorized agents shall conduct its activities in a manner consistent with State water quality standards (including any requirements resulting from compliance with §303(d) of the Clean Water Act) and any other appropriate requirements of State and Federal law. If the NCDWR determines that such standards or laws are not being met (including the failure to sustain a designated or achieved use) or that State or federal law is being violated, or that further conditions are necessary to assure compliance, the NCDWR may reevaluate and modify this certification. [15A NCAC 02B.0200]. Condition 27: The Permittee shall report any violations of this certification to the Division of Water Resources within 24 hours of discovery. [15A NCAC 02B.0506(b)(2)]. Condition 29. Upon completion of the project (including any impacts at associated borrow or waste sites), the NCDOT Division Engineer shall complete the "Certification of Completion Form" to notify the NCDWR when all work included in, he 401 Certification has been completed. [15A NCAC 02H.0502(f)] complete return certification forms online. Condition 30: A copy of this Water Quality Certification shall always be maintained on the construction site. In addition, the Water Quality Certification and all subsequent modifications, if any, shall be maintained with the Division Engineer and the on -site project manager. [15A NCAC 02H .0507(c) and 15A NCAC 02H .0506 (b)(2) and (c)(2)] Greg Daisey discussed 72612-Permit Replacing of Bridge 935 over Branch of Slade Creek 83x3Oft bridge on existing alignment, with an off -set detour in cooperating 140ft of steel sheeting stabilization, overhead power, water line, and telephone line. Special conditions were discussed. David Knoll stated from S.T. Wooten that they would do the sheeting first and use safety fence or orange silt fence for the erosion control. Black silt is already out there near waterline. On January 13, 2020 which will also be the start date of project and they will meet with surveyors. Paul Williams will make call with vegetation and pulling of silt fence up if it's stable. Maintenance will pull up silt fence. TROGDON, III SECRETARY Jay Mombaerts discussed: • Prosecution of work, approved epoxy payment for this work will be incidental. • All asphalt across caps should be saw cut to depth of quarter inch • Scope of work • No major contract line item • DBE 4%, 9% with Florence. • Listing of DBE Subcontractors • State funded but has federal provision • Clearing & Grubbing Method II • Turbidity Curtain Installation • Price Adjustment • Guardrail End Units, Type-TL-2 • There will be a detour; Marshall Gill and I, will reroute the detour and will send a new report to Tim, moving it to the middle of the road Doughty Lane and looping around Swindell Road. Will coordinate with Traffic Services for whatever changes needed. We will talk to Rick West. • There will be an asbestos inspection this week. If there is asbestos, this will affect timing of the project. • We will be using concrete wash house. These minutes are completed as noted. Any discrepancies to the content of the minutes should be provided in writing to the Resident Engineer by 2/17/2020. If no discrepancies are noted, this will indicate concurrence with the minutes as noted.
