Hi. Hope you’re doing well. I can call you, but this email provides some background information.
In November 2019?, I received a call from Adrienne Brookshire (AB) because she received invoices for renewal of both
NCG030665 and NCG030666. AB said she replaced John Peters at US Chemical Storage (facility associated with
NCG030666). To confirm her statement, the state received/processed a name/ownership change for NCG030666 in
November 2019. NCG030666 is paid up. There is some confusion because from looking at records, it appears that in
the past NCG030665 and NCG030666 were both under US Chemical Storage. This is no longer the case because when I
spoke with AB she said she did not know anything about the NCG030665 facility. It appears the NCG030665 facility was
purchased by Craftmaster Furniture. The two permits are for facilities at two different locations:
NCG030665 – 1605 Industrial Drive, Wilkesboro - Craftmaster Furniture (used to be US Chemical Storage) – AB does not
know anything about this facility. John Peters still listed as contact, but he is no longer there.
NCG030666 – 1806 River St, Wilkesboro – US Chemical Storage – Adrianne Brookshire replaced John Peters as contact
NCG030666 appears to be doing what they need to be doing to making necessary changes to abide by their
permit. NCG030665 facility needs followup because this ownership change could have resulted in potentially different
activities at site generating possible need for different permit type (assuming that they still have a stormwater discharge
to surface water that is covered under stormwater program). They also need to pay up if they are still covered.
The following email chain may provide some additional helpful information. I’m more than willing to answer questions
that you may have regarding this situation because we thought it would be good if you could visit site within the next 30
days or so. This has been an unresolved issue for awhile. Again, please contact me with any questions, concerns,

I can try to help. It appears that NCG030666 requested name/ownership change was approved? It is NCG030665 that
is more complicated. The NCG030666 contact did not know anything about situation re: NCG030665. NCG030666 is
paid up, NCG030665 is paid until this year.
We need to get ownership and contact info sorted out for NCG030666. It looks like they submitted a Name/Owner
change form on 10/9 – Suzanne, is that still in the works? Maybe that’s what brought up all these questions in the first
place. I’ve forgotten a few details. Can we sort it out with that request, at least when fees are also paid? We’ll need to
make sure we get their billing contact up to date, too.

I found the hard copy file for NCG030665. It appears that the Certificate of Coverage was renewed in Sept
2015. Permittee has been up to date on annual fees until 2019. They are now overdue. There was a printed email
message from Bill Diuguid that stated WSRO and the Permittee had discussed the possibility of combining NCG030665
and NCG030666. These discussions were prior to Craftmaster Furniture purchasing? The NCG030665 facility.
Why this issue was even brought to my attention was that the new contact (AB) at NCG030666 facility is receiving
correspondence for both 665 and 666 even though she is contact for only 666 facility.
It appears that some digging may need to be done to determine/confirm status of 665 facility which appears to be under
new ownership.

I have NCG030663 and NCG030667 files in the import queue. But nothing in between…so no NCG030665 or
NCG030666.
Have we lost those, or are they in a box that Richard has at StarPoint?
In BIMS, it looks like they are a lot older than last year. I think Bradley just set them up for the automatic renewal last
year – I bet that’s why they look like they were entered so close together. He was helping us with that part.
Bill Diuguid was the reviewer, and he retired a long time ago. If anyone knows where his files are, we might find them
there. Otherwise, we have to go to Central Files! That’s where the original NOI would have gone…

Bradley entered NCG030665 and NCG030666 last October, prior to my arrival. The Permitee is claiming one of the
facilities is not theirs.
