Facility Overview:
The Town of Columbus WWTP is a minor municipal facility with no significant industrial
discharges and no pretreatment program. In the past, industrial discharges did flow to the
WWTP, requiring metals and toxicity monitoring. According to the previous fact sheet, the
pretreatment program has been inactive for 20+ years. Columbus WWTP has a design flowrate
of 0.8 MGD, with a maximum daily flow rate of 0.5 MGD for 2018, as reported in their renewal
application.
Compliance History:
• Most recent permit cycle shows multiple limit violations for fecal coliform and multiple
monitoring violations for TRC, among other violations.
• They've passed over 75% of TOX tests within the last 3 years.
RPA:
The maximum monthly average flow between Oct 2015 and Oct 2018 was approximately
0.240452 MGD.

The 7Q10s used was 2.1 cfs, since there have been no expansions or modifications to the facility.
All previous RPAs dating back to the 1998 renewal have also used 2.1 cfs for 7Q1Os. The 7Q10s
found in BIMS (0.96 cfs) should be used for any future modelling, as stated in a 12/30/1994
memo responding to the Town of Columbus's request for modification of metal limits:
"The 7Q10 flow of 2.1 cfs is used in the equations also. However, in 1993, new flow
information was gotten from USGS that indicates that the flow at Columbus's discharge
point is 0.96 cfs, 55% lower than the flow that has been used. This updated flow will be
used in the future if any expansions or modifications are requested."
As noted in the 1998 renewal, the current IWC is 37%, however 56% should be used for any
future expansion, for the reasons noted above.
• Copper - reasonable potential to exceed water quality standards; applying monthly
monitoring with limits.
• Cadmium — reasonable potential to exceed water quality standards; applying monthly
monitoring with limits.
• Cyanide — reasonable potential to exceed water quality standards; applying monthly
monitoring with limits.
• Lead — No reasonable potential to exceed water quality standards; predicted max > 50%
of allowable concentration for chronic and predicted max < 50% of allowable
concentration for acute, so quarterly monitoring will be continued.
• Silver — reasonable potential to exceed water quality standards; applying monthly
monitoring with limits.
• Zinc — no reasonable potential to exceed water quality standards; predicted max > 50% of
allowable concentration, so quarterly monitoring will be continued
• Mercury — no reasonable potential, but a limited data set; annual monitoring and
reporting will be continued to gather more data for next renewal.
Changes from previous permit to draft:
• Added regulatory citations.
• Updated Supplement to Cover Sheet #1 information to include facility address and #2 to
include stream index and HUC.
• Added facility grade in A(1).
• Updated eDMR footnote in A(1) and language in A(6).
• Removed former footnote #4 stating "Monitoring should be conducted in conjunction
with toxicity monitoring" as this is no longer required.
• Added Hardness to A(1) Effluent table to monitor in conjunction with hardness
dependent metals already being monitored (copper, cadmium, silver, lead and zinc), as
per Implementation of Instream Dissolved Metals Standards 2016 Memo. Sampling in
effluent and upstream.
• Added footnotes #6 and #7 regarding effluent and instream hardness sampling.
• Added Section A(3) compliance schedule for new metal limits and monitoring, with
corresponding footnote #8 in Section A(1).

• Updated components list to include new automatic spiral screen compactor, vortex grit
removal, correct number of aeration basin floating aerators, new 12' deep clarifier, new
sludge holding tank, two RAS/WAS recirculating pumps, and chlorination and
dechlorination upgraded to flow proportional.
• Added Copper, Cadmium, Cyanide and Silver limits and monthly monitoring to A(1)
Effluent table, as new RPA showed a reasonable potential to exceed WQS.
• Facility reclassified to Class III Biological WPCS, so the following parameters have
increased monitoring frequency to 3/Week per 15A NCAC 02B .0508: BOD, TSS,
Ammonia Nitrogen, Fecal Coliform, DO, TRC and pH.
• Added Wastewater Management Plan (WWTP) special condition to the permit for a
solids removal plan and sheet -pile wall assessment, as requested by ARO in their
04/08/2019 Staff Report.
Chances from draft to final:
• Technical Correction: Monitoring frequency for Temperature was erroneously increased
to Daily. This has been corrected in the final version to 3/Week in the A(1) Effluent table
per 15A NCAC 02B .0508 for a Class III Biological WPCS.
• Added footnote #9 in A(1) regarding reporting and compliance for Total Cyanide.
• Added Section A(5) Effluent Mercury Analysis and updated corresponding footnote #4 in
A(1).
Comments received on draft permit:
• Permittee requested the monitoring frequency for Temperature be changed from Daily (in
the draft) to 3/Week. This was an error which has been corrected for the final version of
the permit renewal.

Form 2A has been developed in a modular format and consists of a 'Basic Application Information" packet and
a "Supplemental Application Information" packet. The Basic Application Information packet is divided into two
parts. All applicants must complete Parts A and C. Applicants with a design flow greater than or equal to 0.1
mgd must also complete Part B. Some applicants must also complete the Supplemental Application
Information packet. The following items explain which parts of Form 2A you must complete.
BASIC APPLICATION INFORMATION:
A. Basic Application Information for all Applicants. All applicants must complete questions A 1 through A 8 A treatment
works that discharges effluent to surface waters of the United States must also answer questions A 9 through A 12
B. Additional Application Information for Applicants with a Design Flow > 0.1 mgd. All treatment works that have design
flows greater than or equal to 0 1 million gallons per day must complete questions B 1 through B 6
C. Certification. All applicants must complete Part C (Certification)
SUPPLEMENTAL APPLICATION INFORMATION:
D. Expanded Effluent Testing Data. A treatment works that discharges effluent to surface waters of the United States and
meets one or more of the following criteria must complete Part D (Expanded Effluent Testing Data)
1 Has a design flow rate greater than or equal to 1 mgd,
2 Is required to have a pretreatment program (or has one in place), or
3 Is otherwise required by the permitting authority to provide the information
E. Toxicity Testing Data. A treatment works that meets one or more of the following criteria must complete Part E (Toxicity
Testing Data)
1 Has a design flow rate greater than or equal to 1 mgd,
2 Is required to have a pretreatment program (or has one in place), or
3 Is otherwise required by the permitting authority to submit results of toxicity testing
F. Industrial User Discharges and RCRA/CERCLA Wastes. A treatment works that accepts process wastewater from any
significant industrial users (SIUs) or receives RCRA or CERCLA wastes must complete Part F (Industrial User Discharges and
RCRA/CERCLA Wastes) SIUs are defined as
1 All industrial users subject to Categorical Pretreatment Standards under 40 Code of Federal Regulations (CFR) 403 6 and
40 CFR Chapter I, Subchapter N (see instructions), and
2 Any other industrial user that
a Discharges an average of 25,000 gallons per day or more of process wastewater to the treatment works (with certain
exclusions), or
b Contributes a process wastestream that makes up 5 percent or more of the average dry weather hydraulic or organic
capacity of the treatment plant, or
c Is designated as an SIU by the control authority
G. Combined Sewer Systems. A treatment works that has a combined sewer system must complete Part G (Combined Sewer
Systems)

The Town of Columbus WWTP land application is permitted
under WQ#0016247, Synagro Western Piedmont, dated
November 17, 2017. Residuals from Columbus WWTP are
hauled to approved sites totaling 313.6 acres, which contain a
mix of pasture, hay, and row crop. Residuals are monitored for
non -hazardous characteristics, nutrients and metals, and lime
stabilized to a PH of 12 for 24 hours to satisfy pathogen and
vector requirements.

In reviewing the DMR data for the Town of Columbus WWTP, we have noticed some potential
errors and inconsistencies with some of the data. We are requesting the lab sheets for the above
months for further review. The values reported for Cadmium, Cyanide, and Lead are reported in the
units of µg/l, however we believe this data was submitted in incorrect units. The values reported for
Silver, Zinc, and Copper are reported as both µg/1 and mg/l. For example, DMR data for Copper in
June 2014 reports data in the units of mg/1, however September 2014 to March 2016 reports data in
µg/1 and June 2016 and onward reports data in mg/1. Similarly, the data for Zinc and Silver from
June 2014 to March 2016 were reported in µg/1, however June 2016 and onwards have been
reported in mg/l. If these values have been reported in error, please submit revised DMRs to the
Division.
The NC 2007-2014 Water Quality Standard.(WQS) Triennial Review was approved by the NC
Environmental Management Committee (EMC) on November 13, 2014. The US EPA subsequently
approved the WQS revisions on April 6, 2016 with some exceptions. The NC Division of Water
Resources NPDES Permitting Unit is required to implement the new dissolved metal standards in
all permits public noticed after April 6, 2016. The metals of concern for the Town of Columbus
WWTP are Total Copper, Total Silver, Total Zinc, Total Cadmium, Total Cyanide, Total Lead, and
Total Mercury.
This information is required in order to continue reviewing the permit renewal request. Please
provide the requested information with 45 calendar days [per 15A NCAC 211.0107(b)]. If you have

Facility was recently updated with the appropriate classification (WW-III). Ricky McGuinn has passed his Grade
II; however, is listed as an OIT potentially due to needing additional operational experience. Waiting for
clarification from Operator Certification to determine if an interim Back-up is required.
2. Are the design, maintenance and operation of the treatment facilities adequate for the type of waste and disposal

If no, please explain: When the Aeration Basin was divided when flows were reduced, the ability to remove
solids was also removed and/or no lonjZer accessible from the basins active side. Solids have continued to build
within the basin and need to be removed to allow the facility to operate as designed. Additional solids storage
capacity may also be warranted.
Description of existing facilities: Mechanical screw auger, vortex grit removal, aeration basin, dual circular
clarifiers, aerobic digester/sludge holding tank, chlorine contact chamber with ,gas chlorine and liquid calcium
thiosulfate (dechlor) and ultrasonic flow meter.

Please summarize any findings resulting from this review: Three upsets were reported to the ARO last ,year
during hey rains. This may be due to the excessive number of solids currently in the facility. Does not appear to
be related to size of chlorine contact chamber and ability to clean this out. Excessive solids may also be
contributing to spikes in fecal on occasion. Additional organic matter suspended in the effluent could be reducing
the effectiveness of the chlorine treatment during heavy flows.
Provide input to help the permit writer evaluate any requests for reduced monitoring, if applicable.

If yes, please explain: The facility needs to assess the current hydraulic capabilities within the Aeration Basin,
determine a means of solids removal from the Aeration Basin and determine if, during normal operational

Here is some additional wording I've added (more just to ensure they manage the aeration basin
moving forward) since they currently dispose most of the biosolids through land application most of
the solids would be managed ok without the additional input from the basin.

I'd love to get more of your input on the wording for the curtain wall assessment condition in the
permit. I'm assuming ARO would like an engineer to assess the wall and have the facility propose
plans for repair, but is there anything else you'd like to specifically address? Also if you have any
comments or suggested changes for the Biosolids removal plan language included in the WWMP
condition, just let me know! I want to make sure this permit fully addresses the items that ARO is
concerned about for this facility.

Many years ago, we had to develop a modular special condition for submission of Wastewater
Management Plans (WWMPs). The original shell is attached.
Starting with that, I culled it down to create a WWMP condition for Columbus. I was able to create
wording for the solids component, but you & Mikal will need to develop the wording for the curtain
study. Tell the permittee what you want done, and what information they should submit.
The completed WWMP condition can then be added to the permit. It is enforceable; failure to
submit the WWMP is a permit violation.
Keep the shell for future reference. When there are performance -related conditions to put in a
permit, start with it. However, be aware that someone in the RO and here must review the WWMP
when it's submitted.

Should the expiration date be in the year 2024? We are under the impression that permits are
valid for 5 years.
o Compliance & Expedited Permitting Unit Response: Permit can be valid up to 5
years, but we follow a basin schedule for the expiration dates. For NC0021369, since
it's in the Broad River Basin, sub -basin 03-08-02, the expiration should be July 31st,
2023. The previous permit expired July 31 st, 2018.
Pages 4-5: The paragraph that begins on page 4 and flows to page 5 read as follows in the
previous permit:
All toxicity testing results required as part of this permit condition will be entered on the
Effluent Discharge Monitoring Form (MR-1) for the months in which tests were
performed. If reporting pass/fail results using the parameter code TGP3B, DWR Form
AT-1 (original) is sent to the below address. If reporting Chronic Value results using the
parameter code THP3B, DWR Form AT-3 (original) ...
It is not clear why mention of submitting an AT-1 form was omitted from this version of the
permit.
o Compliance & Expedited Permitting Unit Response: We are currently using the most
up-to-date template language that we know of for our permits with toxicity testing,
with different template language for the different types of testing required. The
language used in Section A. (2.) for permit NCO021369 is from the "Freshwater —
Chronic Limit TGP313 Quarterly-03-2013" document on the S-drive, located at:
S:\SWP-Point Source\NPDES\Aquatic Toxicity. Please let us know if there is more
updated language available that we should be using instead.
