III. Certification Statement:
I Chandra C. Farmer, P.E. certify to the best of my knowledge that the addition of
the volume of wastewater to be permitted in this project has been evaluated along the route to the receiving
wastewater treatment facility and that the flow from this project is not anticipated to cause any capacity
related sanitary sewer overflows or overburden any downstream pump station en route to the receiving
treatment plant under normal circumstances, given the implementation of the planned improvements
identified in the planning assessment where applicable. This analysis has been performed in accordance
with local established policies and procedures using the best available data. This certification applies to
those items listed above in Sections I and II plus all attached planning assessment addendums for which I
am the responsible party. Signature of this form certifies that the receiving collection system or treatment
works has adequate capacity to transport and treat the proposed new wastewater.

Given reasonably expected conditions and planning information, there is sufficient justification to allow
this flow to be permitted, without a significant likelihood of over -allocating capacity in the system
infrastructure.
I understand that this does not relieve the collection system owner from complying with G.S. 143-
215.67(a) which prohibits the introduction of any waste in excess of the capacity of the waste disposal
system.

Dear Mr. Broome:
On February 22, 2005, the Division of Water Quality received a flow reduction request for
residences in Johnston County. The letter requests an allowable flow rate of 80 GPD per residential
bedroom based on the evaluated data for three similar existing subdivisions.
The water use data provided between November 2003 and October 2004 with daily readings
taken during November 2004 was analyzed by central office and regional office staff with additional
information requested. Based upon this data and the County response to our additional information
requests, the Division hereby approves the use of 80 GPD per residential bedroom, with a minimum
of 160 gpd for one and two bedroom units in all applicable non -discharge permit applications.
Flow shall be incremented by 80/gpd for each bedroom above two. Regardless of the adiusted
design dail wastewater flow rate at no time shall the wastewater flows exceed the effluent limits
defined in the NPDES vermit for the treatment facility or exceed the capacity of the sewers
downstream of any new sewer extension or service connections . A pet'mit modification and fee is
required to apply this flow reduction to any existing permits where flows have been allocated.

Proiect Narrative:
Neuse Haven Phase 2 will have 12 new triplex units, 8 of which will be three bedroom units and 4 will be
four bedroom units, and +/-5601f of new 8" gravity sewer. Requested flow is 3,200 GPD. The project will
flow to an existing public gravity system owned and maintained by Johnston County. The flow will end
up at the Central Johnston County WWTP permit NC0030716.

We are in the process of reviewing the subject application and require additional information.
VI.6. "Yes" is marked for a flow reduction, however a copy of the flow reduction approval letter was not

On the Flow Tracking form it column E shows a negative available capacity for Clayton K5 Pump Station. As
instructed in Column E, a Planning Assessment Addendum should be attached for any PS where the available
