NCG210000. Since both names are indicated on the application, would you like "Perfect Choice Gifts" or "K&V Pallets"

Please see attached draft COC, specifically the two yellow high lighted sections and indicated which of the two you
would like to appear, or do you want Perfect Choice as authorized to discharge from facility at K&V Pallets.
