portions of this submittal package, inclusion of these materials under my signature and seal signifies that I have reviewed this
material and have judged it to be consistent with the proposed design.
NOTE - In accordance with General Statutes 143-2I5.6A and 143-215.613, any person who knowingly makes any false
statement, representation, or certification in any application package shall be guilty of a Class 2 misdemeanor, which may
include a fine not to exceed $10,000, as well as civil penalties up to $25,000 per violation.

On behalf of Nash County, we are submitting one (1) original and one (1) copy
of the following documents related to the above referenced project for your
review and approval:

This project will install approximately 2426 linear feet of 8" SDR-35 PVC,
approximately 715 linear feet of 8" ductile iron gravity sewer main. The project
site is currently not served with sewer. Gravity sewer will be extended from the
pump station on US 264 to the back of the vacant shell building on Middlesex
Corporate Parkway. No services are proposed in this project hence it is a zero -
flow project. The gravity sewer is proposed to serve the shell building when the
future tenants occupy the building.
IK — The Town of Middlesex will own and maintain the proposed gravity sewer after
construction. All new sewer improvements have been designed in accordance
with the 15A NCAC 02T requirements.

This email is confirm that Raleigh has an interlocal agreement to provide wastewater treatment services to the Town of
Middlesex. Since there is no flow currently assigned the permit to allow us to determine if there is capacity at the plant
and within the interlocal agreement, it does not appear that a flow acceptance statement is appropriate at this time.
However, we are in agreement to allow the non -discharge permit to be issued for construction of the proposed sewer
main as requested, with the understanding that we will have the opportunity to review the proposed wastewater flows
from the project and determine if there is adequate capacity in the plant and agreement before a service connection is
made.
I hope this response is satisfactory to allow the issuance of the non -discharge permit for the construction of the
proposed sewer main extension.

(1) specifically refer to the project, regardless whether capacity has been purchased through an
intergovernmental agreement of contract;
(2) signify that the owner of the receiving collection system or treatment works has adequate capacity to
transport and treat the proposed new wastewater, and
(3) shall be dated within 12 months from the dote of submitting the application.
This letter shall not obviate the need for the downstream sewer capacity calculations.

We are preparing to draft the permit but have a couple of questions.
I. Will the proposed sewer line discharge to a sewer line owned by the Town of Middlesex? If so, we'll need
written confirmation from the Town of Middlesex approving this.
2. We need confirmation from the City of Raleigh accepting the waste to be treated at the Little Creek WWTP
(Zebulon).
To expedite issuance of the permit, we'll accept an email confirmation from the City of Raleigh approving the
acceptance of the waste at the Little Creek WWTP, and an email from the Town of Middlesex approving acceptance of
the waste in their sewer line.
3. The narrative for this project notes the Town of Middlesex will own and maintain the proposed gravity sewer
after construction. An ownership change form will need to be submitted for this to occur, transferring
ownership from Nash County to the Town of Middlesex.
Weil continue drafting the permit upon submittal of information in items 1 & 2 above.

period the Town of Middlesex and Nash County will complete an ownership change form to transfer ownership of the
sewer main to the town.

We are preparing to draft the permit but have a couple of questions.
1. Will the proposed sewer line discharge to a sewer line owned by the Town of Middlesex? If so, we'll need
written confirmation from the Town of Middlesex approving this.
2. We need confirmation from the City of Raleigh accepting the waste to be treated at the Little Creek WWTP
(Zebulon).
To expedite issuance of the permit, we'll accept an email confirmation from the City of Raleigh approving the
acceptance of the waste at the Little Creek WWTP, and an email from the Town of Middlesex approving acceptance of
the waste in their sewer line.
3. The narrative for this project notes the Town of Middlesex will own and maintain the proposed gravity sewer
after construction. An ownership change form will need to be submitted for this to occur, transferring
ownership from Nash County to the Town of Middlesex.
We'll continue drafting the permit upon submittal of information in items 1 & 2 above.
