I N C January 8, 2020 U.S. Army Corps of Engineers Attn: Mr. David Bailey Raleigh Regulatory Field Office Wilmington District U.S. Army Corps of Engineers 3331 Heritage Trade Drive, Suite 105 Wake Forest, North Carolina 27587 North Carolina Division of Water Resources Attn: Ms. Sue Homewood Winston-Salem Regional Office - 401 Wetlands Unit 450 West Hanes Mill Road Winston Salem, North Carolina 27105 Reference: Restoration and Post Construction Monitoring 2019 Annual Monitoring Report (Year 1) NOV-2017-OP-0008 Triad Business Park Old Greensboro Road Extension Kernersville, Guilford County, North Carolina Pilot Project 1010.2 Dear Mr. Bailey and Ms. Homewood: Mr. Brian Hall, representative of TDO Land Holdings, LLC, received a "Notice of Violation" from the U.S. Army Corps of Engineers (USACE) and North Carolina Department of Environmental Quality - Division of Water Resources (NCDEQ-DWR) dated September 20 and 28, 2017. Pilot Environmental, Inc. (Pilot), on behalf of TDO Land Holdings, LLC, prepared a remedial plan to address the notice of violation that was approved by your offices on December 19 and 21, 2017. Remedial actions, including restoration of both the stream and wetland, concluded in November 2018 when vegetative plantings were installed. Pilot visited the site multiple times in 2019 to visually inspect the restoration areas to determine needed repairs or maintenance. Pilot conducted the last site visit on November 20, 2019 to conduct an annual inspection of the stream and wetland restoration areas. Installed in -stream structures, planted vegetation and other visual observations of the channel design were inspected. The stream was observed to be within its designed channel and installed structures were observed to be properly functioning. No structures were observed to be failing or in need of repairs or maintenance. Additionally, herbaceous vegetation has become well established and planted tree saplings and live stakes were observed to be healthy and located throughout.
