Re: Compliance Evaluation Inspection — Federal Bureau of Prisons, FCC Butner
Dear Mr. Zimmerman,
This response is to demonstrate our dedication to ensuring a safe and environmentally
responsible correctional institution. As identified in Section 8 of the November 14, 2019
`Compliance Inspection Report' the following deficiencies were listed:
1. One manhole ring was loose and needed replacement to prevent infiltration or accidents
from occurring.
2. One manhole lid could not be opened and needs to be addressed to ensure that proper
inspection can occur.
3. Facility staff stated that they are anticipating installing pole barriers around the manholes
because they are often impacted by mowing machinery while crews maintain the road
shoulder.
Corrective actions:
1. The mortar has been replaced on the three ground level manhole rings along NC Rt. 75.
2. The identified manhole lid was removed and the ring and cover were cleaned of all debris
to allow for inspections. All manholes were verified for physical accessibility.
3. Yellow warning bollards have been installed adjacent to the three ground level manholes
along NC Rt. 75.
4. Additional inspection requirements have been added to the institution's Total
Maintenance System database (TMS) requiring staff to visually inspect all `deemed
collection system' manholes on a weekly basis. Also, a semi-annual inspection was
created for staff to verify that each manhole can be physically opened and accessed.
Attached you will find pictures and a copy of the added inspection criteria. I hope this
adequately addresses your agency's concerns. If there is additional information or documentation
