We are requesting a modification to existing permit No. WQ0037031 Mod for the
Fortune Place II project to rectify an omission. The original submittal for this phase left
off a leg of sewer which has now been installed. We respectfully request the addition of
3271f of 8" sewer to be added on to the permit. No additional flow is needed.
Please find enclosed one (1) original and one (1) copy of the fast -track application and a
check for the application fee. Please review for approval and contact us with any
