To Whom It May Concern:
This submittal is to satisfy the five (5) day written notification requirement in our NPDES Permit
(Part II Section E.6.). On December 13th 2019, we received analytical results for Outfall 005 for
the third fluoride sample of 2.4 mg/l. The previous two results were 2.1 mg/I and 1.6 mg/I.
The three samples resulted in a monthly average of 2.033mg/I, our permit is 1.8 mg/I.
On 2019 December 13', Randall Kiser of my staff verbally reported this November exceedance
to Roberto Sheller of the Mooresville Regional Office of the Department.
We will keep the Department apprised of any further findings and additional actions that we
