REGAN Secretary LINDA CULPEPPER Director NORTH CAROLINA Environmental Quality January 2, 2019 CERTIFIED MAIL # 7017 1450 0001 6450 1099 RETURN RECEIPT REQUESTED Travis Rakes 1185 Bent Twig Dr. King, NC 27021 SUBJECT: Compliance Evaluation Inspection Certificate of Coverage #NCG550506 — General Permit 550000 Single -Family NPDES Wastewater Treatment System 1185 Bent Twig Dr. King, NC 27021 Stokes County Dear Mr. Rakes: On December 31, 2019, staff of the North Carolina Division of Water Resources' Winston-Salem Regional Office (DWR) performed a compliance inspection of the single-family NPDES wastewater treatment system located at the subject property. This compliance inspection was conducted by DWR staff person Mr. Justin Henderson. Inspection of the wastewater treatment system reflects non-compliance with Permit No. NCG550506. The following deficiencies require your attention and action: 1) The Certificate of Coverage NCG550506, for the subject wastewater treatment system is expired, therefore in accordance with Permit Condition Part II. Section B. (10.) the permittee is not authorized to discharge after expiration. While the residence appeared to be vacant currently, the wastewater treatment system should remain inactive, without a discharge to surface waters of the State, until the Certificate of Coverage is renewed. 2) Failure to remit payment for the Annual Administering and Compliance Monitoring Fee, authorized by Permit Condition Part II. Section B. (14.), for the following years: 2012, 2013, 2014, 2015, 2016, 2017, 2018, 2019. Permit Condition Part I. Section D. (L) requires an adequate supply of wastewater grade tablets (calcium hypochlorite) be maintained in the chlorinator to ensure continuous and proper disinfection. b) The cascade aerator nor the discharge pipe could be located during this inspection. All wastewater treatment system components shall be located and then request made to this office for re -inspection. c) It was noted during this inspection that the subject residence was listed for sale. Please be aware per Permit Condition II. Section E. (4.) this permit is not transferable to any person without the prior written notice to and approval from the Director in accordance with 40 CFR 122.61. d) Permit Condition Part I. Section C. requires annual effluent monitoring for the parameters contained therein. Permit Condition Part I. Section D. (5.) requires that the effluent monitoring data be maintained on -site for three years. This Office is formally requesting that copies of the effluent monitoring conducted for the preceding three years be submitted within 30-days receipt of this Notice. Failure to abide by the conditions contained in your permit may subject the permittee to enforcement action by DWR in accordance with North Carolina General Statute 143-215.6A to 143-215.6C and/or revocation of this permit.
