Operating a treatment facility, discharging wastewater, or discharging specific types of stormwater to waters of the State
without a valid NPDES permit will subject the responsible party to a civil penalty of up to $25,000 per day. Enforcement
action will be certain for persons that have voluntarily relinquished permit coverage when, in fact, continuing permit
coverage was necessary. If, in retrospect, you feel the site still requires permit coverage, you should notify this office
immediately. Furthermore, if in the future you wish to again discharge to the State's surface waters, you must first apply
for and receive new coverage under an NPDES permit.

PROCEDURES, REGARDING FIELD LOGS, LABORATORY DATA SHEETS AND
SAMPLES
Procedures Rewarding Field Lags. Laboratory Data Sheets and Samples
In the process of obtaining and testing samples and preparing this report, procedures are followed
that represent reasonable and accepted practice in the field of soil and foundation engineering.
Specifically, field logs are prepared during performance of the drilling and sampling operations
which are intended to portray essentially field occurrences, sampling locations, and other
information.
Samples obtained in the field are frequently subjected to additional testing and reclassification in
the laboratory by more experienced soil engineers, and differences between the field logs and the
final logs exist.
The engineer preparing the report reviews the field and laboratory logs, classifications and test
data, and his judgment in interpreting this data, may make further changes.
Samples are taken in the field, some of which are later subjected to laboratory tests, are retained
in our laboratory for sixty days and are then discarded unless special disposition is requested by
our client. Samples retained over a long period of time, even if sealed in jars, are subject to
moisture loss which changes the apparent strength of cohesive soil generally increasing the
strength from what was originally encountered in the field. Since they are then no longer
representative of the moisture conditions initially encountered, an inspection of these samples
should recognize this factor.
Field logs and laboratory data sheets have not been included in our engineering reports because
they do not represent the engineer's final opinions as to appropriate descriptions for conditions
encountered in the exploration and testing work. Results of the laboratory tests are generally
described in the appendices, shown on the boring logs and/or described in the extent of the
report, as appropriate.

On, December 19,2008, 1 made a site visit to Kinston Lumber Yard. The company was closed, and
operations were no longer active. I found no petroleum, oils, lubricants, or saw dust piles exposed.
My recommendation is to rescind the SW permit NCG210262.

Hi Jeff -
I've attached the rescissions requests that we have not gotten a response on yet.
We would really like to go ahead and rescind these by the end of the month if we
haven't heard anything from your region. I think you've responded on all of the
ones for your counties, but I'm not sure who else in the region to send these to.

Due to the termination of all manufacturing operations at its Kinston, North Carolina facility,
Coastal Lumber Company requests that the above referenced storm water permit be classified
as inactive
Due to the lack of qualifying activities, Coastal Lumber Company suspended storm water
sampling at the facility during the 2001 calendar year. Operations ceased at this facility at the
effective October 1, 2000.
At this time, Coastal Lumber Company has no plans to resume the operations covered by this
permit.

In response to your renewal application for continued coverage under the General Permit
NCG040000, the Division of Water Quality (DWQ) is forwarding herewith the General Permit
NCG210000 which is a new General Permit to cover most of the Timber Products Industry. You
have now been given coverage under NCG210000 and your coverage under NCGO40000 is hereby
terminated. This permit is issued pursuant to the requirements of North Carolina General Statute
143-215.1 and the Memorandum of Agreement between North Carolina and the US Environmental
Protection Agency dated December 6, 1983.
The following information is included with your.permit package:
■ A copy of the stormwater general permit NCG210000.
■ A new Certificate of Coverage under general permit NCG210000.
■ A Stormwater Pollution Prevention Plan Certification Form. This form certifies that you have
developed and implemented the Stormwater Pollution Prevention Plan (SPPP) required in your
permit (both NCGO40000 and NCG210000). This form must be completed and returned to the
Division within 30 days of receipt of this letter. DO NOT send the SPPP with the signed form.
■ Five copies of Analytical Monitoring forms.
■ Five copies of Qualitative Monitoring forms.
■ A copy of a Technical Bulletin on the stormwater program which outlines program components
and addresses frequently asked questions.
Your certificate of coverage is not transferable except after notice to DWQ. The Division of Water
Quality may require modification or revocation and reissuance of the certificate of coverage.
This permit does not affect the legal requirements to obtain other permits which may be required by
DWQ or permits required by the Division of Land Resources, Division of Air Quality, Coastal Area
Management Act or any other Federal or Local governmental permits that may be required.
