In accordance with your application for a discharge permit received on December 4, 2013,.we
are forwarding herewith the subject certificate of coverage (COC) to discharge under the subject state -
NPDES general permit. This permit is issued pursuant to the requirements of North Carolina General
Statute 143-215.1 and the Memorandum of Agreement between North Carolina and the US
Environmental Protection Agency dated October 15, 2007 (or as subsequently amended).
Piease note that this COC does not cover discharges from the irrigation of raw material
stockpiles, a wastewater from this site. This condition is a result of the information presented in your
application. The site plan provided was lacking adequate stormwater measures to capture runoff from
the site, in the event of an accidental spill, or discharges from wetting of aggregate stockpiles occur. The
surface flow paths shown, and possible sampling points on the site plan were not accurately shown. Staff
from our Mooresville Regional Office may wish to schedule an inspection soon after your facility begins
operation to ensure the permit conditions are being met..
Please note that permit requirements in Part 11, Section A, numbers 1-5, and Part 11, Section B,
number 1 are no longer valid per NC Session Law 2011-394. However, note that Part 11, Section A,
number 6, and Part II, Section B, numbers 2-10 are unaffected by this Session Law. Those sections will
remain in effect for the duration of this permit term. Please be reminded that your facility rnust notify the
appropriate DWQ Regional Office at least 72 hours in advance of operation of your wastewater treatment
facility.
This certificate of ccveraf;e is not transferable except upon approval of the Division of Energy,
Mineral, and Land Resources (DIEMLR). If the facility changes ownership or is closed, DEMLR may
require modification or revocation and reissuance of the certificate of coverage. This permit does not
affect the legal requirements to obtain other permits which may be required by DEMLR, or permits
required by other federal, state, or local authorities.

/703/1705 Morgan Mill Road LLC (a.k.a bluedot Ready Mix) proposes to construct a Ready Mix
Concrete Plant at the subject location. Included in plans for this plant is a concrete and water recycling
system. The system is designed with enough freeboard to contain the 24 hour 25 year storm event with
1' additional freeboard. This system has been submitted to Nathaniel D. Thornburg for review.
The proposed system is for capturing process water generated from loading trucks at the ready mix
concrete batch plant. The recycle system consists of a sloped washout bay, 2 sloped settling chambers,
overflow basin ,pump basin and a pump for recycled water. Process water that enters the system is
generated by spills during truck water tank filling, rinsing of the outside of the mixer truck/drum and
rinsing the inside of the drum at the end of the day. The area (4,932 square feet) that these operations
take place in are grade separated from the balance of the site by curbs, walls and ramps. The area
draining to the recycle basin was minimized to reduce the amount of stormwater runoff collected.
The only process water that does not flow to the recycle system is runoff from wetting of aggregates.
The aggregates will be water with municipal or well water. No cement laden water, high ph, will be
used to water aggregates. This water will be discharged through the walls of the bunker block aggregate
bunkers. The runoff from wetting of aggregates will contain less fines than the stormwater runoff. Sand
and coarse aggregates for concrete are limited by ASTM C33 to 5% of the material passing a 200 sieve.
A 200 sieve has 200 openings per inch. This equates to a 0.005" opening, which is twice the size of the
0.0025" sieve. Typical tests on material that will be utilized at this plant have a lower percent passing
the 200 sieve than required by ASTM C33. Sand ranges between 1.8% and 2% passing the 200 sieve.
Coarse aggregate is below 1% passing a 200 sieve. The runoff from the aggregate stockpiles will not
contain a sediment load as high as stormwater running across the ground. Runoff from aggregate storage
piles watered with clean water, ie. Municipal or well water, is clear water.
The aggregate stockpiles will be wet with city water during the summer months. All aggregates were
tested for Ph level by David Melson (QC — Blue Dot). The results are shown on attachment 1. City

water is typically in distributed with a Ph between 6.5 and 8. Municipalities are required by permit to
maintain the water at this Ph. Runoff from wetting of aggregate stockpiles will not cause an exceedance
of Ph or Suspended Solids in water discharged from the site.
A stormwater erosion control plan has been submitted to the City of Monroe.

inspected the site on Jan 2, 2014. It is in a commercial / light industrial area. Across the creek is a Concrete Supply mix
site. All stormwater
drains to the creek at the rear of the site. There is a good buffer between the usable portion of the site and the top of
bank. Please see pies.
Also, I do not see a proposed monitoring/ sampling location on the site plan.

In accordance with your application for a discharge permit received on December 4, 2013, we
are forwarding herewith the subject certificate of coverage (COC) to discharge under the subject state -
NPDES general permit, This permit is issued Pursuant to the requirements of North CaroLna General
Statute 143-215.1 and the Memorandum of Agreement between North Carolina and the US
Environmental Protection Agency dated October 19, 2007 (or as suhsequently amended),
Please note that this COC does not cover discharges from the irrigation of raw material
stockpiles, a wastewater from this site. This condition is a result of the information presented in your
application. The site plan provided was lacking adequate stormwater measures to capture runoff from
the site, in the event of an accidental spill, or discharges from wetting of aggregate stockpiles occur. The
surface flow paths shown, and possible sampling points on the site plan were not accurately shown. Staff
from our Mooresville Regional Office may wish to schedule an inspection soon after your facility begins
operation to ensure the permit conditions are being met.
Please note that permit requirements in Part 11, Section A, numbers 1-5, and Part I1, Section B,
number 1 are no longer valid per NC Session Law 2011-394. However, note that Part 11, Section A,
number 6, and Part II, Section B, numbers 2-10 are unaffected by this Session Law. Those sections will
remain in effect for the duration of this permit term. Please be reminded that your facility must notify the
appropriate DWQ Regional Office at least 72 hours in advance of operation of your wastewater treatment
faa:il-ty.
This certificate of coverage is not transferable except upon approval of the Division of Energy,
Mineral, and Land Resources (DEMLR). If the facili-ty changes ownership or is closed, DEMLR may
require modification or revocation and reissaance of the certificate of coverage. This permit does not
affect the legal requirements to obtain other permits which may he required by DEMLR, or permits
required by other federal, state, or local authorities.

Dear Ms. Georgoulias,
1703/1705 Morgan Mill Road LLC (a.k.a bluedot Ready Mix) proposes to construct a Ready Mix
Concrete Plant at the subject location. Included in plans for this plant is a concrete and water recycling
system. The system is designed with enough freeboard to contain the 24 hour 25 year storm event with
F additional freeboard. This system has been submitted to Nathaniel D. Thornburg for review.
The proposed system is for capturing process water generated from loading trucks at the ready mix
concrete batch plant. The recycle systern consists of a sloped washout bay, 2 sloped settling chambers,
overflow basin ,pump basin and a pump for recycled water. Process water that enters the system is
generated by spills during truck water tank filling, rinsing of the outside of the mixer truck/drum and
rinsing the inside of the drum at the end of the day. The area (4,932 square feet) that these operations
take place in are grade separated from the balance of the site by curbs, walls and ramps. The area
draining to the recycle basin was minimized to reduce the amount of stormwater runoff collected.
The only process water that does not flow to the recycle system is runoff from wetting of aggregates.
The aggregates will be water with municipal or well water. No cement laden water, high ph, will be
used to water aggregates. This water will be discharged through the walls of the bunker block aggregate
bunkers. The runoff from wetting of aggregates will contain less fines than the stormwater runoff. Sand
and coarse aggregates for concrete are limited by ASTM C33 to 5% of the material passing a 200 sieve.
A 200 sieve has 200 openings per inch. This equates to a 0.005" opening, which is twice the size of the
0.0025" sieve. Typical tests on material that will be utilized at this plant have a lower percent passing
the 200 sieve than required by ASTM C33. Sand ranges between 1.8% and 2% passing the 200 sieve.
Coarse aggregate is below 1% passing a 200 sieve. The runoff from the aggregate stockpiles will not
contain a sediment load as high as stormwater running across the ground. Runoff from aggregate storage
piles watered with clean water, ie. Municipal or well water, is clear water.
The aggregate stockpiles will be wet with city water during the summer months. All aggregates were
tested for Ph level by David Melson (QC — Blue Dot). The results are shown on attachment 1. City

water is typically in distributed with a Ph between 6.5 and S. Municipalities are required by permit to
maintain the water at this Ph. Runoff from wetting of aggregate stockpiles will not cause an exceedance
of Ph or Suspended Solids in water discharged from the site.
A stormwater erosion control plan has been submitted to the City of Monroe.

(2) For a partnership or sole proprietorship: by a general partner or the proprietor, respectively; or
(3) For a municipality, state, federal, or other public agency: by either a principal executive officer or ranking elected official.

I inspected the site on Jan 2, 2014. It is in a commercial / light industrial area. Across the creek is a Concrete Supply mix
site. All stormwater
drains to the creek at the rear of the site. There is a good buffer between the usable portion of the site and the top of
bank. Please see pits.
Also, I do not see a proposed monitoring/ sampling location on the site plan.
