In accordance with your application for a discharge permit received on December 14,
2011, we are forwarding herewith the subject certificate of coverage (COC) to discharge under
the subject state — NPDES general permit. This permit is issued pursuant to the requirements of
North Carolina General Statute 143-215.1 and the Memorandum of Agreement between North
Carolina and the US Environmental Protection Agency dated October 15, 2007 (or as
subsequently amended).
Please note that NCG 140000 contains effluent limitations for process wastewater
discharges (see Part IV, Section D). Wastewater discharges must meet the requirements of
these effluent limitations. An erceedence of any of these limitations is a violation of permit
conditions.
Please note that groundwater standards in 15A NCAC 02L and 02T regulations must also
be met for recycle systems or any discharges to groundwater.
Please note that this COC does not cover discharges from the irrigation of raw
material stockpiles, a wastewater, from this site. This condition is a result of the information
presented in your application.
Please take notice that this certificate of coverage is not transferable except after notice to
the Division of Water Quality. The Division of Water Quality may require modification or
revocation and reissuance of the certificate of coverage. This permit does not affect the legal
requirements to obtain other permits which may be required by the Division of Water Quality or
permits required by the Division of Land Resources, Coastal Area Management Act or any other
federal or local governmental permit that may be required.

Section B, number 1 are no longer valid per NC Session Law 2011-394. However, note that Part
I1, Section A, number 6, and Part II, Section B, numbers 2-10 are unaffected by this Session
Law. Those sections will remain in effect for the duration of this permit term. Please be
reminded that your facility must notify the appropriate DWQ Regional Office at least 72 hours in

1 have a few questions regarding the NOI submitted for the subject facility.
1. The NOI in items 9 and 19 state that the wetting of the aggregate stockpiles will not discharge; and the
site will therefore have stormwater only discharges. However, the plans state that the stockpile wetting will
drain across the site to the detention basin; which would then be considered a commingled ww and sw
discharge. Please verify which is correct. It is not a huge issue, but it can affect how the COC will be written.
2. You stated on item 17 of the NOI that the closed -loop recycle system proposed will meet the
requirements in 02T .1000. Have you contacted the Aquifer Protection Section regarding this? They will be the
ones to do any review/approval on closed -loop recycle systems under their program; and any discharge from the
closed -loop recycle system will NOT be covered by the subject NCG14 permit.

Please see NOI attached. I'm also sending you separately a hardcopy of the full-size plans.
We have received an application for coverage of a new readymix facility under NCG 14 from 158 Intercraft Drive, LLC
(blue dot readi-mix). The facility drains to an UT to Fourth Creek (class C) in Statesville (Iredell County). They have
vehicle maintenance, but no haz. waste. They state in the NOI that they will have a closed -loop recycle system that
meets the 02T .1000 rules (I need to confirm that they have contacted the Aquifer Protection Section about this). Also,
the NOI states sw discharges only, but the plans show the aggregate pile wetting draining to the detention basin and
discharging (I will be contacting them to verify). Does the MRO have any concerns about issuing this facility a COC for
this general permit?

This office has reviewed the subject erosion and sedimentation control plan. We find Elie plan to
be acceptable with modifications and herby issue this Letter of Approval with Modifications.
Modifications must be received in this office no later than two weeks prior to the project start
(late, failure to submit modified plans can result in a violation and fine under the Iredell County
Soil Erosion and Sedimentation Control Ordinance. The Modifications required for Approval arc
listed on (lie attached page. The Certificate of Approval issued by Iredell County must be posted
at the job site. This plan approval sliall expire three (3) years following the date of this approval
as is required by'Title 15A NCAC 4B.0129.
Title 15A NCAC 4B 0118(a) requires that a copy of the approved erosion control plan be on file
at the job site. Any amendments and/or revisions to the approved plan must be reviewed and
approved prior to implementation in order for the site to remain in compliance. Failure to submit
plans for approval may result in administrative actions and/or penalties as indicated in the Iredell
County Soil Erosion and Sedimentation Control Ordinance. Also, this letter gives notice as
required by G. S. 113A-61.1(a) of our right of periodic inspection to insure compliance with the
approved plan.

lredell County's Sedimentation Pollution Control Program is performance -oriented, requiring
protection of existing natural resources and adjoining properties. If, following the
commencement of this project, it is determined that the erosion and sedimentation control plan is
inadequate to meet the requirements of the Iredell County Soil Erosion And Sedimentation
Control Ordinance and the Sedimentation Pollution Control Act of 1973, North Carolina General
Statute It 3A-51 through 66), this office may require revisions to the plan and implementation of
the revisions to insure compliance with the Act.
Acceptance and approval of this plan is conditioned upon your compliance with Federal, State,
and County water duality laws, regulations, and rules. This approval does not supersede any
other peniiit or approval.
Please be aware that your project will be covered under the NPDES General Stor7rrwater- Permit
NCGO1000 (Constitution Activities) that was issued with the original erosion control plan
approval. You are responsible for becoming familiar with all of the requirements for compliance
regarding the general permit.
Please note that this approval is based in part on the accuracy of tine information provided in the
Financial Responsibility Form, which you provided. You are required to file an amended form if
there is any change in the information included on the form. In addition, it would be helpful if
you notify this office of the proposed starting date for this project. Please notify us if you plan to

This office has reviewed the subject erosion and sedimentation control plan. We find the plan to
be acceptable with modifications and herby issue this Letter of Approval with Modifications.
Modifications must be received in this office no later than two weeks prior to the project start
date. Failure to submit modified plans can result in a violation and fine under the tredeli County
Soil Erosion and Sedimentation Control Ordinance. The Modifications required for Approval are
listed on the attached page. The Certificate of Approval issued by Iredell County must be posted
at the job site. This plan approval shall expire three (3) years following the date of this approval
as is required by Title 15A NCAC 4B.0129.
Title 15A NCAC 4B 01 18(a) requires that a copy of the approved erosion control plan be on file
at the job site. Any amendments and/or revisions to the approved plan inust be reviewed and
approved prior to implementation in order for the site to remain in compliance. Failure to submit
plans for approval may result in administrative actions and/or penalties as indicated in the Iredell
County Soil Erosion and Sedimentation Control Ordinance. Also, this letter gives notice as
required by G.S. 113A-61.1(a) of our right of periodic inspection to insure compliance with the
approved plan.

Iredell County's Sedimentation Pollution Control Program is performance -oriented, requiring
protection of existing natural resources and adjoining properties. If, following the
coinmencernent of this project, it is determined that the erosion and sedimentation control plan is
inadequate to meet the requirements of the Iredell County Soil Erosion And Sedimentation
Control Ordinance and the Sedimentation Pollution Control Act of 1973, North Carolina Cenci -at
Statute 113A-5I tlu-ough 66), this office may require revisions to the plan and implementation of
the revisions to insure compliance with the Act. .
Acceptance and approval of this plan is conditioned upon your compliance with Federal, State,
and County water duality laws, regulations, and rules. Tlvs approval sloes not supersede any
other permit or approval.
Please be aware that your project will be covered under the NPDES General Stormwater Permit
NCGO 1000 (Construction Activities) that was issued with the original erosion control plan
approval. You are responsible for becoming familiar with all of the requirements for compliance
regarding the general permit.
Please note that this approval is based in part on the accuracy of the information provided in the
Financial Responsibility Form, which you provided. You are required to file an amended form if
