In accordance with your application for a discharge permit received on October 5, 2006,
we are forwarding herewith the subject certificate of coverage (COC) to discharge under the
subject state — NPDES general permit. This permit is issued pursuant to the requirements of
North Carolina General Statute 143-215.1 and the Memorandum of Agreement between North
Carolina and the US Environmental Protection Agency dated May 9, 1994 (or as subsequently
amended):
Please take notice that this certificate of coverage is not transferable except after notice to
the Division of Water Quality. The Division of Water Quality may require modification or
revocation and reissuance of the certificate of coverage. This permit does not affect the legal
requirements to obtain other pen -nits which may be required by the Division of Water Quality or
permits required by the Division of Land Resources, Coastal Area Management Act or any other
federal or local governmental permit that may be required.
An Authorization to Construct treatment facilities .has been issued concurrently with
this COC. The Mooresville Regional Office, telephone number [phone], shall be
notified at least forty-eight (48) hours in advance of operation of the installed facilities so that an
in -place inspection can be rnade. Such notification to the regional supervisor shall be made
during the normal office hours from 8:00 a.m. until 5.00 p.m. on Monday through Friday,
excluding State Holidays.
Upon completion of construction and prior to operation of this permitted facility, a_
certification must be received from a professional engineer certifying that the Permitted
facility has been installed in accordance with the NPDES Permit, the associated
Authorization to Construct, and the approved plans and specifications. Please mail the
certification (attached) to the Storinwater Permitting Unit, 1617 Mail Service Center, Raleigh,

to Construct (ATC) prior to construction per 15A NCAC 2H .0 13 8 & .0139. You must submit,
in triplicate, plans/specifications and design calculations, stamped and sealed by a professional
engineer, with a letter requesting an ATC to the Stormwater Permitting Unit, 1617 Mail Service
Center, Raleigh, NC 27699-1617.1

I wanted to see if you have had time to review the most recent submittal for Thomas Concrete. I was anticipating
a phone call or email last week with your comments. Again, the most recent submittal sent, I have re -graded the
site to minimize the amount of storm/wastewater entering the settlement pit. From the calculations I have
performed, the settlement pit/detention pond will hold the 25 yr. storm without discharge. Please review the plan
as soon as you have time and let me know if I need to change/add anything to the plan. If necessary, I can take a
day and come meet with you at your office.

I have made the grade changes as we discussed yesterday. With regsrd to the batch plant wastewater, Thomas would like to
gravity flow this to the settlement pit. I wM go over this with them today. However. I would like you to look at the revised
calca. New Rouse pond size MX30'X 12.5'. I could not reduce the depth with the requirament of i' freeboard and assu,mintg
an iuiital water depth of 2' (1,680 eu.8., 12,600gal.).
Assuming 1' of depth in the serilcment pit is based ou 10 trucks washing out per day at 150 gallons each (1.500 gal). I ft
storage in settlement pit - +/-3,713 gallons. I full bay=+/-3.442 gallons
As discussed, we will set a 50yr. weir at the same elevation as the invert $nm the scvJcment pit.
I am meeting with Thomas Concrete today to go over the revisions/additions

Hi Matt,
We've entered the Thomas Concrete - Monroe submittal into our database as of October
5, 2006, but I haven't started my review, yet. I expect to start at the end of next
week. If the submittal is complete, my review will only take a day. So, if the
submittal is ok, Thomas should have a permit sometime the week of November 13 - 17.
I know it seems we're awfully slow in processing the Thomas submittal. Our unit is
working to improve our turn around times, and I am especially concerned with the
ready -mixed concrete submittals. Unfortunately, on occasions in the recent past we
have exceeded the 90-day turn around required of us in North Carolina's rules at 15A
NCAC 2H .0112(a). And there are still some other applications on my desk for which I
won't be able to make the required turn around. But we are attempting to expedite
the ready -mixed concrete submittals, including yours for Thomas. I appreciate your
patience and the fact that your inquiries into the status of the Thomas submittal
have been very professional.

Yes, that is it. It doesn't include talcs for the ditch from the batch
plant, however that should be very straight forward and only add to the
drainage area a little. Please let me know if you have any comments.
