Sullins Director April 27, 2011 Subject: Compliance Evaluation Inspection PLT Concrete Services — Selma _ NPDES Stormwater Perm"if.NCG-140362::�) Johnston County Dee Freeman Secretary I, Mitch Hayes, performed the subject inspection on April 12, 2011. The assistance of Bill Anderson with this inspection was greatly appreciated. Please find attached with this letter the Basin Wide Information Management System (BIMS) checklist. Below is a list of findings and recommendations developed from the inspection. 1. The current permit was issued and became effective August 01, 2009. The General Permit NCG140000 will expire June 30, 2011. There is no requirement to submit a new Notice of Intent or renewal request unless directed by the Division of Water Quality. 2. The facility consists of an infiltration basin, a five -chamber drum wash recycle system, drainage grass swales on both side of the property, a water hose area for washing off the outside of trucks, and a water detention pond designed to hold a 25-year, 24-hour rainfall event. The facility is designed to not discharge any process wastewater water. There has been no discharge from the facility. Fuel and admixture tanks have secondary containment with locked gate valves. There was some water seepage on the end of the secondary containment wall. This should be repaired. All excess water on the property is routed to grass swales located on each side of the property. Water in the swales flows to a detention pond. The pond appeared to have four feet of freeboard. There was no indication of overflow from the pond. 4. A review of records indicate the following: illicit non-stormwater discharge inspection was completed February 01, 201 1; employee training was documented December 28, 2010; containment drainage was documented April 04, 2011, facility site inspection was last completed September 28, 2010; qualitative monitoring was completed October 14, 2010; analytical monitoring was completed in April 2011 however, final results are pending. He and his consulting engineer, Ron Sutton, had met with Ken Pickle in March, 2006 for a couple of hours with drawings, but had left without leaving any materials. So there was no NOI on file with the state. Nisely responded to the call with a visit to the site. Ken and Myrl both agree that it is a well designed plant, based upon and improving on the successful designs used in PLT plants in Wilson and Rocky Mount. Nonetheless, Mr. Proctor had run ahead of protocol by building without approval. Nisely has numerous pictures taken during this inspection. Please find attached with this letter the Basin Wide Information Management System (BMS) checklist. Below is a list of findings and recommendations developed from the inspection. I. The current permit was issued and became effective January 16, 2008. The General Permit NCG140000 will expire 07/31/2009. There is no requirement to submit a new Notice of Intent or renewal request unless directed by the Division of Water Quality. 2. The facility consists of an infiltration basin, a five -chamber drum wash recycle system, drainage ditches, and a water detention pond designed to hold a 25-year, 24-hour rainfall event. The facility is designed to not discharge any process wastewater water. There has been no discharge from the facility. 3. Fuel and admixture tanks have secondary containment with locked gate valves. Water is visually inspected before discharge. All excess water on the property is routed to the grass swells located on each side of the property. Water in the swells is discharged to the water detention pond. The pond was approximately one - quarter full. Excess concrete is poured into blocks or poured onto the ground. A large area between the pond and the wash out bays is covered with stacks of broken concrete slabs. This combined with the poured concrete on the ground has increased the amount of impervious surface area, which may increase stormwater flow and cause the detention pond to overflow. 4. The water detention pond is constructed with an overflow pipe and emergency spill way. Analytical monitoring is not required if the water detention pond discharges as a result of a I0-year — 24 hour precipitation event. 5. The Stormwater Pollution Prevention Plan (SPPP) was 'reviewed. The SPPP is well written and meet all requirements with the exception of implementing and documenting annual employee training on proper spill response and cleanup procedures and preventative measures. Site inspection was conducted 04/30/2007 and 11/13/2007. Inspection of all containment areas was last completed on 04/30/2008. Non-stormwater discharge inspection was completed 05/3I/2008.
