I spoke to Mr. Waters, who is the owner of Roanoke-Chowan Ready Mix, Inc. in Bertie County. I explained to him that
the permit NCG140297 expired on July 31, 2009 and that it would be the best for all of us if we would rescind that
permit instead of NCG140331. He agreed and said that it must have been some sort of typo in rescission request letter
where they requested rescission of NCG140331.
Since we have two permits for same facility, we in WaRO don't have any problems with rescission of permit NCG140297.
i hope this is sufficient for you to rescind the permit. If not, please iet me know how can I help.

In accordance with your application for discharge permit received on October 7, 2002, we are forwarding herewith
the subject certificate of coverage to discharge under the subject state - NPDES general permit. This permit is issued
pursuant to the requirements of North Carolina General Statute 143-215 .1 and the Memorandum of Agreement between
North Carolina and the US Environmental Protection agency dated December 6, 1983.
The stormwater permit you are receiving, NCG140000, does not permit wasting concrete, dumping excess
concrete, directly into storm sewer outfalls or into waters of the state. Any facility which either dumps excess concrete or
washes excess concrete into storm sewers or waters of the state will be operating in direct violation to both the terms of
this permit and the North Carolina General Statutes. Such a discharge shall be considered an illegal discharge and may
subject the owner to enforcement actions in accordance with North Carolina General Statutes 143-215.6A
If any parts, measurement frequencies or sampling requirements contained in this permit are unacceptable to
you, you have the right to request an individual permit by submitting an individual permit application. Unless such
demand is made, this certificate of coverage shall be final and binding.
Please take notice that this certificate of coverage is not transferable except after notice to the Division of Water
Quality. The Division of Water Quality may require modification or revocation and reissuance of the certificate of
coverage.
This permit does not affect the legal requirements to obtain other permits which may be required by the Division
of Water Quality or permits required by the Division of Land Resources, Coastal Area Management Act or any other
Federal or Local governmental permit that may be required.

This Certificate of Coverage shall remain in effect for the duration of the General Permit.

Dear Permittee:
Your facility is currently covered for stormwater discharge under General Pert -nit NCG 140000, This permit will
expire on July 31, 2004. North Carolina General Statute 143.215.1(c) requires that an application for permit
coverage renewal be filed in a timely manner. A prior notice of renewal was sent for this site on December 1,
2003. To date, no response has been received by the Division of Water Quality (DWQ). In order to assure your
continued coverage under the general permit, you must apply to DWQ for renewal of your permit coverage.
Enclosed you will find a general permit renewal application form. The application must be completed and returned
by April 19, 2004 in order to assure continued coverage under the general permit.
Failure to request renewal by April 9, 2004 can result in a civil assessment of at least $250.00. Larger penalties
may be assessed depending on the delinquency of the request. Discharge of stormwater from your facility without
coverage under a valid stormwater NPDES permit would constitute a violation of NCGS 143-215.1 and could
result in assessments of civil penalties of up to $10,000 per day.
