must be completed and returned to the Division within 30 days of receipt of this letter.
DO NOT SEND the SPPP with the signed form.
* Five copies of the Analytical Monitoring form and five copies of the Qualitative Monitoring form
* A copy of a Technical Bulletin for the general permit which outlines changes in the permit, key requirements,
and addresses frequently asked questions
* A Certificate of Coverage
Your coverage under this general permit is not transferable except after notice to DWQ. The Division may require

During the NCG 140000 permit renewal process, Mr. John Holding of Thomas Concrete of Carolina
brought to the Division attention the fact that the NOI he had filed in November of 1997, had never
been acted upon. He provided copies of the signed NOI and cancelled check. He also noted that the
facility was inspected and approved by Steve Mitchell of the RRO in 1997.
Steve Mitchell was contacted on 02/09/99. He confirmed that he had conducted a site visit in 1997
and he endorsed issuing permit coverage.
To address this situation the Division is expediting the issuance of the Certificate of Coverage for
this facility, with an expected issue date of February 12, 1999.
