Cleanout (MD). Report more than one type if the waste -stream is commingled.
If an effluent limit is exceeded twice in a row, the permittee is required to institute monthly monitoring for that parameter for six months, unless DWQ RO staff
notifies you to continue monitoring.
pH limits are 6-9 S.U. for wastewater discharges to freshwaters, and 6.8-8.5 S.U. for discharges to saltwaters.

but instead is subject to benchmarks and provisions of Part IV, Section A, including the Tiered Response Action.
Flow rate can be measured continuously or calculated. Flow limits for wastewater discharges to HQW waters shall be set to 50% of the Summer 7Q10 Flow as per 15A
NCAC 0213.0224. Permittees who discharge wastewater to HQW waters shall obtain a Summer 7Q10 flow and report this information to DWQ. if the permittee cannot
obtain a Summer 7Q10 flow for the receiving waters at the discharge location, the permittee shall notify DWQ, and the DWQ Regional Office may require an annual flow
report on a case -by -case basis.

The plant does not have a water heater for mixing concrete during qol& weather. However, the
facility does have several plastic totes and tanks that contain additives used in the concrete to provide
different characteristic (i.e., strength, curing time, etc). The additives are located outside and behind a
containment wall.
7. Compliance with Specific Permit Conditions and Limitations:
a. Condition A3. — 15A NCAC 2D .0515, "Particulate Control Requirement". Emission source_ s
must meet the allowable emission rates as defined by the process rate.
Observed:' There are no recordkeeping or reporting requirements. The facility was determined
to be in compliance with this condition during the Air Permit application process.
b. .Condition AA.— 15A NCAC 2D .0521, "Control of Visible Emissions". The facility is limited to
20 percent opacity.
Observed:. The facility was.loading a mixer.truck at the time of, inspection and emissions,
remained below 20% opacity. Compliance with this stipulation was indicated.
c. Condition A.5. --'.'Notification Requirement". Notify DAQ of excess emission that last more than
four hours that result.from a malfunction, a breakdown of process or control equipment or any
other abnormal conditions.

with this stipulation was indicated.
d.- 'Condition A.6 —15A NCAC 2D .0540, "Control'from Fugitive Dust Emission Sources" required
the facility not to cause or allow fugitive dust emissions to cause or contribute'to substantive
'complaints or excess visible emissions beyond the property boundary.
Observed: According to Mr. Palmer and a review of MRO files indicate no complaints have been
received regarding fugitive dust. There was no fugitive dust observed during this inspection.

Observed:' Internal inspections are conducted monthly. The last internal inspection on all of the
bagfilters was conducted on April 12, 2016. The logbooks were properly maintained.
Compliance with this permit condition was indicated.
8. GACTIMACT Review
The facility does not have any reciprocating internal combustion engines subject to NESHAP 4Z.
The facility does -have a pump for moving water and several air compressors, however, the equipment
is electric and does'not appear to be subject to NESHAP 4Z.
