Enclosed is a copy of the Compliance Evaluation Inspection (CEI) report for the inspection
conducted at the subject facility on April 24, 2008, by Mr. Ron Eubanks and Mr. Jason Klingler of
Charlotte -Mecklenburg Storm Water Services. This inspection was conducted as part of a cooperative
working agreement between Mecklenburg County and the Division of Water Quality. In addition, the
inspection was conducted on behalf of the City of Charlotte in compliance with the City's NPDES Permit,
NCS000240, Part 11 Section H. Thank you for your assistance and cooperation during the inspection.
Overall, the inspection found that the State Street Plant operated by Southern Concrete Materials
has developed and implemented a Stormwater program and this Office commends the permittee's efforts to
ensure compliance with your NPDES permit.
The enclosed report should be self-explanatory; however should you have any questions

Inspection Summary. -
Facility Description:
Southern Concrete Materials State Street Facility covers approximately 7.5 acres and consists of two (2) plants. One is
a central mix plant while the other is a dry batch plant. Ready -mix concrete is produced in an enclosed area in the
central mix plant. The dry batch plant depends upon trucks to mix the concrete Ready mix concrete is produced at the
site by combining sand, gravel, cement, fly ash, water, and admixtures. Some of the raw materials are stored in
stockpiles while other materials are stored in silos to protect them from slormwater runoff. The chemical admixtures
are stored in bulk chemical storage tanks. After the ready -mix concrete is produced, the product is removed from the
site by truck and distributed to off -site construction projects.

The permitteelfacility does not have any prior compliance issues with the NC Division of Water Quality and the
following was found in the Charlotte Mecklenburg Storm Water Services (CMSWS) compliance files.
On August 16, 2004, CMSWS investigated a discharge into an unnamed tributary of Stewart Creek from Southern
Concrete Materials State Street facility. During the investigation, CMSWS personnel observed a gray colored plume
entering Stewart Creek from an unnamed tributary of Stewart Creek. There was also a build up of concrete residue on
the stream bed of both the tributary and main channel of Stewart Creek. The discharge was identified to be from
Southern Concrete Materials. Southern Concrete Materials addressed the discharge by repairing the impacted storm
drain system at the site.
On November 29. 2007 CMSWS investigated a complaint from the City of Charlotte Street Maintenance group
regarding a discharge of concrete slurry to the municipal storm drain system from Southern Concrete Materials State
Street facility. No discharge was observed by CMSWS personnel during a site inspection on November 30, 2007.
CMSWS personnel noted that the stormwater management practices implemented by Southern Concrete Materials in
October 2004 appeared to be working properly.
Industrial Operations/City of Charlotte Stormwater Ordinance Review;
Central Mix Concrete Plant —
Ready mix concrete is produced in a covered area in the central mix plant. The plant consists of overhead aggregate
bins, cement and fly ash silos, and conveying and weighing equipment. Fine and coarse aggregates are delivered by
trucks, dumped into a drive -over hopper or stockpiled an site. Materials from stockpiles are loaded into the hopper and
conveyed to the concrete plant. Cement and fly ash are delivered to the site by railcar and/or tanker trucks and
pneumatically pumped into the overhead silos. Admixtures used in the concrete are delivered to the site in tanker
trucks and pumped into bulk chemical storage tanks in a covered area. Admixtures are then pumped into the concrete
in varying proportions depending upon the mix design requirements. Once the raw materials are fed into the mixer and
combined to form ready mix concrete, the final product is removed from the site by truck and distributed to off -site
construction projects,
Dry Batch Concrete Plant --
The dry batch plant uses the same raw materials as the central mix plant. However, the dry batch plant depends upon
the ready mixed concrete trucks to mix the concrete. The admixtures for the dry batch plant are located in an outdoor
storage area with secondary containment. The cement, fly ash, sand, stone, water, and admixtures are proportioned
by weight and discharged into the concrete trucks for mixing and subsequent delivery.
Truck Wash Area —
After loading, drivers wash dried products from the trucks in the truck wash area. The truck wash water discharges into
a collection basin, which allows the solids to settle. The wash water is recycled for reuse in plant operations.

Mixer Drum Washout/ Return Concrete Area -
At the end of the day concrete clinging to the inside of the drums must be washed out. Returned concrete is used to
construct concrete blocks or it is placed in small piles to dry. When the concrete has set it can be handled as a solid
waste instead of a liquid waste.

Comment: The site plan should be updated to indicate the location of the receiving
stream (unnamed tributary to Stewart Creek). The stormwater BMPs (storm drain inlet
protection, silt fencing in drainage channel, etc.) should also be identified on the site
plan.
Southern Concrete has installed 18-inch risers around the storm drain inlets at the site
to prevent process water from entering the storm sewer system. The area around the
risers should be routinely checked and cleaned as needed.

Comment: Southern Concrete has installed silt fencing below the storm drain outfall
on State Street (below the site). The silt fencing appeared to be working effectively to
prevent excess concrete residue from entering the tributary to Stewart Creek during
storm events. The storm drainage channel and the area in front of the silt fencing
should be routinely checked and cleaned as needed.

Water Quality (DWQ) is forwarding herewith the reissued Stormwater general permit. This permit is reissued
pursuant to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement
between the state of North Carolina and the U.S. Environmental Protection Agency, dated December 6, 1983.
The following information is included with your permit package:
• A new Certificate of Coverage
• A copy of General Stormwater Permit NCG 140000
• A copy of a discharge monitoring report form
• 5 copies of the qualitative monitoring report form
Your coverage under this general permit is not transferable except after notice to DWQ. The Division may require
modification or revocation and reissuance of the Certificate of Coverage. This permit does not affect the legal
requirements to obtain other permits which may be required by the Department of Environment and Natural
Resources, or relieve the permittee from responsibility for compliance with any other applicable federal, state, or
local law, rule, standard, ordinance, order, judgment, or decree.

* A Stormwater Pollution Prevention Plan (SPPP) Certification Form. Completion of this form is required to
certify that you have developed and implemented the SPPP as per the conditions of the permit. This form
must be completed and returned to the Division within 30 days of receipt of this letter.
DO NOT SEND the SPPP with the signed form.
* Five copies of the Analytical Monitoring form and Five copies of the Qualitative Monitoring form
* A copy of a Technical Bulletin for the general permit which outlines changes in the permit, key requirements,
and addresses frequently asked questions
* A Certificate of Coverage
Your coverage under this general permit is not transferable except after notice to DWQ. The Division may require

In accordance with your application for a stormwater discharge permit received on December 2, 1994, we
are forwarding herewith the subject certificate of coverage to discharge under the subject state - NPDES general
permit. This permit is issued pursuant to the requirements of North Carolina General Statute 143-215 .1 and the
Memorandum of Agreement between North Carolina and the US Environmental Protection agency dated
December 6, 1983.
The stormwater permit you are receiving, NCG140000, does not permit wasting concrete, dumping excess
concrete,. directly into storm sewer outfalls or into waters of the state. Any facility which either dumps excess
concrete or washes excess concrete into storm sewers or waters of the state will be operating in direct violation to
both the terms of this permit and the North Carolina General Statutes. Such a discharge shall be considered an
illegal discharge and may subject the owner to enforcement actions in accordance with North Carolina General
Statutes 143-215.6A
If any parts, measurement frequencies or sampling requirements contained in this permit are unacceptable to
you, you have the right to request an individual permit by submitting an individual permit application. Unless
such demand is made, this certificate of coverage shall be final and binding.
Please take notice that this certificate of coverage is not transferable except after notice to the Division of
Environmental Management. The Division of Environinenta. Management may require modification or revocatior
and reissuance of the certificate of coverage. - _
This permit does not affect the legal requirements to obtain other permits which may be required by the
Division of Environmental Management or permits required by the Division of Land Resources, Coastal Area
Management Act or any other Federal or Local governmental permit that may be required.
