Please find enclosed the revised Certificates of Coverage for the 17 facilities that changed
ownership from Concrete Supply Co to Concrete Supply Co., LLC. The terms and conditions
contained in the General Permit remain unchanged and in full effect. These revised Certificates
of Coverage are issued under the requirements of North Carolina General Statutes 143-215.1
and the Memorandum of Agreement between North Carolina and the U.S. Environmental

In response to your renewal application for continued coverage under general permit NCG140000, the Division of
Water Quality (DWQ) is forwarding herewith the reissued stormwater general permit. This permit is reissued
pursuant to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement
between the state of North Carolina and the U.S. Environmental Protection Agency, dated December 6, 1983.
The following information is included with your permit package:
* A copy of general stormwater permit NCG 140000
* A Stormwater Pollution Prevention Plan (SPPP) Certification Form. Completion of this form is required to
certify that you have developed and implemented the SPPP as per the conditions of the permit. This form
must be completed and returned to the Division within 30 days of receipt of this letter.
DO NOT SEND the SPPP with the signed form.
* Five copies of the Analytical Monitoring form and five copies of the Qualitative Monitoring form
* A copy of a Technical Bulletin for the general permit which outlines changes in the permit, key requirements,
and addresses frequently asked questions
* A Certificate of Coverage
Your coverage under this general permit is not transferable except after notice to DWQ. The Division may require
modification or revocation and reissuance of the Certificate of Coverage. This permit does not affect the legal
requirements to obtain other permits which may be required by DENR or relieve the permittee from responsibility
for compliance with any other applicable federal, state, or local law, rule, standard, ordinance, order, judgment, or
decree.

Chapter 143, Article 21 of the North Carolina General Statutes authorizes and directs the
Environmental Management Commission of this Department to protect and preserve the water
resources of the State. The Division of Water Quality has the delegated authority to enforce adopted
pollution control rules and regulations.
On January 9, 2008, Mrs. Olivia Edwards and Mr. Daniel Royal of the Mecklenburg County
Water Quality Program (MCWQP) conducted an investigation of the subject facility and the adjacent
sections of a storm water conveyance to Stewart Creek. This investigation was conducted as part of a
cooperative working agreement between Mecklenburg County and the Division of Water Quality and
was in response to a complaint of a wash water/concrete slurry discharge from the subject facility

As a result of this investigation, the following conditions were found,
1. Discharge of wash water and concrete slurry to a storm water conveyance located on an
adjacent property to the Concrete Supply West Plant.
pH readings of 10.82 standard units (SU) was collected from the wash water at the inlet
of the storm water catch basin. This exceedance is a violation of your NPDES General
Permit NCG140054, which states, "The pH shall not be less than 6.0 SU not, greater
than 9.0 SU".
3. A pH reading of 10,97 SU was taken in the storm water conveyance approximately 100
linear feet downstream of the storm water outfall.

e readings indicate that the wash water and concrete slurry discharged from the Concrete
Supply West Alan%Ad adversely impacted a storm water conveyance to Stewart Creek. This constitutes

a violation 6f NC6S'.143-215. I (a)(6) and 15A NCAC 213 .0211(3)(g).
To comply'with the requirements of NCGS 143-215.1 and 15A NCAC 213.0211(3)(g) it will
$e.necessary to"Amediately discontinue all illegal discharge activity; to collect, remove and properly
dispose of the'discharge material, to restore all affected areas as nearly as may be to the conditions
existi g priof�to;the discharge; and to implement corrective actions to eliminate the potential for
recurrent illegaP discharges.
It is requested that you respond, in writing, to this Notice, indicating the measures taken to
identify the source of the discharge, to remove the discharge, to prevent future discharges, and to
restore all affected areas. The response should also indicate the disposal location for any removed
discharged material, contaminated soils or waters, and include a copy of the analytical documentation
for the site remediation, if applicable. Please address your response to Ms. Marcia Allocco by no
later than March 14 2008.
This letter is to notify you that the Mooresville Regional Office of the Division of Water
Quality is considering sending a recommendation for enforcement action to the Director of the
Division of Water Quality. The recommendation concerns the violations of NCGS 143.-215.1 and
15A NCAC 2B .021 1(3)(g) for causing or permitting waste, directly or indirectly, to be intermixed
with waters of the State in violation of the N.C. Water Quality Standards and/or permit conditions.
Violators are subject to penalties of up to $25,000 per day per violation. If you have an explanation
for the violations that you wish to present, please include it in the requested response.
Your explanation will be reviewed and if an enforcement action is still deemed appropriate,
your explanation will be forwarded to the Director alpng with the enforcement package for his
consideration.
