Hi Johnie, nice to hear from you.
OK, so I think you are telling me that the 3509 Ridge Rd address is no longer the address recognized by the Matthews
Post Office, right? I think that you are telling me that the same piece of the surface of Planet Earth now is most
efficiently located by the address, "440 Seaboard Dr", right? OK, then.
I presume we share the concern that the outdated address may someday cause a problem in efficient communication;
or maybe it already has. OK, then.
You know, sometimes in the larger perspective the most expeditious thing to do is for the bureaucrat to do all the
paperwork, and on the other hand sometimes the more efficient approach is for the permittee to do just a portion of
the paperwork. With an emphasis in this case on conserving some of my time, here's what I propose if I understand
the circumstances correctly.
• You download our Name Change format http://portal.ncdenr.org/web/wq/ws/su/npdessw
• This form wasn't constructed for the current circumstance at NCG140039, but we can force -fit it to work enough
to correct our database address.
• On the form, you fill out items I; II c and d; III d and e; V; and finally VI --- fill out the dotted line with, "as per
communications with Ken Pickle December 17, 2012", and strike through the all caps, "THIS APPLICATION WILL
BE RETURNED ...."; and provide just the one signature under Applicant's Certification.
• Send it to Attention: Ken Pickle at the address shown on the form.
• I'll get it and will remember this exchange between us; the paperwork will be clear to me upon inspection, and I
can pass it on to administrative staff who can make'the necessary change in our database. (I'm a particularly
bad one'to go into our data base to make changes. The combination of my general ignorance, and the
database's fragility wrt wrong information, or even the right information entered the wrong way, means that
the administrative staff here regularly volunteer to enter information for me, lest I mess up the works.)
With this approach, I can shepherd the address Change through our organization, without me actually damaging our
database.
If I don't correctly understand the circumstances, please set me straight.

* A Stormwater Pollution Prevention Plan (SPPP) Certification Form. Completion of this form is required to
certify that you have developed and implemented the SPPP as per the conditions of the permit. This form
