This permit is reissued pursuant to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement between the state of North Carolina and the U.S. Environmental Protection Agency, dated October 15, 2007 (or as subsequently amended). The following information is included with your permit package: • A new Certificate of Coverage • A copy of General Permit NCG140000 • A copy of the Technical Bulletin for the General Permit • Two copies of the Discharge Monitoring Report (DMR) Form for Stormwater • Two copies of the DMR Form for Wastewater • Two copies of the Qualitative Monitoring Report Form The General Permit authorizes discharges of stormwater and/or wastewater, and it specifies your obligations with respect to discharge controls, management, monitoring, and record keeping. Please review the new permit to familiarize yourself with all the changes in the reissued permit. If your Certificate of Coverage (COC) restricts authorization to discharge stormwater only (or other limited terms), you must contact the Stormwater Permitting Unit if you would like to request a modification to your COC. The first sample period of your permit begins July 1, 2011. However, because of the delayed mail -out of permit packages, your facility is expected to attempt to sample during the first sampling period, and must comply with sampling requirements beginning during the second sampling period. Your facility must sample a representative storm event between January 1 and July 30, 2012 (for stormwater sampling), and/or sample wastewater between October 1, 2011 and December 31, 2011 (or, if applicable, report "No Flow", as outlined in Part V, Section E). Additionally, your facility has 6 months from the time of receipt of the permit to update SPPP to current standards. The more significant changes in the General Permit since your last Certificate of Coverage include the following (please note the names and numbers of sections have been changed from the previous permit revision): Part 11: • Authorization to Construct (ATC) language was added to the permit and adapted to fit NCG140000. Part III: • The Stormwater Pollution Prevention Plan (SPPP) section has been updated to the most current language of our permits. Some additional conditions specific to this industry have been added to the SPPP language. Part IV: • Sections A & B: Revised provisions require stormwater discharges be sampled twice per year (instead of annually), and sampling results shall be compared to new benchmark values. Tier Two requires that the permittee institute monthly monitoring (reported to the appropriate Regional Offices) instead of twice -per -year monitoring. Monthly monitoring shall be done until three consecutive monitoring events show no benchmark exceedences. • Sections A & B: This permit now contains a provision that four exceedences of any benchmark will trigger increased DWQ involvement in the permittee's stormwater management and control actions (Tier Three). DWQ may direct the permittee to apply for an individual permit, institute specific monitoring, or may direct the implementation or installation of specific stormwater control measures. • Sections A & B: Failure to perform analytical stormwater monitoring requires that the permittee begin a monthly sampling scheme for both industrial stormwater discharges and/or stormwater discharges from VMA areas (where applicable). After 6 months of monthly sampling, the permittee may return to semi- annual sampling, unless otherwise specified by DWQ. • Sections A & B: A lower TSS benchmark of 50 mg/I for HQW, ORW, PNA and Tr Waters was added to this permit for these more sensitive waters. • Section B: The monitoring parameter Oil & Grease has been replaced by the parameter Total Petroleum Hydrocarbons. • Sections A, B & C: Inability to sample due to adverse weather must be reported in the SPPP. • Sections A, B & C: The terminology "Representative Storm Event" has been replaced by "Measurable Storm Event" in response to comments from industry groups. A measurable storm event is a storm event that results in an actual discharge from the permitted site outfall. The previous measurable storm event must have been at least 72 hours prior, unless the permittee is able to document that a shorter interval is representative for local storm events during the sampling period, and obtains written approval from the local DWQ Regional Office. • Section C: Qualitative monitoring must now be conducted semi-annually and is tied to the same measurable storm event as the analytical monitoring. • Section C: If the permittee fails to respond effectively to problems identified by qualitative monitoring, the permittee may be required by DWQ to perform corrective action. • Section D: This section has now been consolidated to one section with one set of combined tables for all types of authorized wastewater discharges. • Section D: The wastewater analytical monitoring schedule has been set to quarterly. • Section D: TSS limits have been revised for PNA, Trout and HQW waters per rule requirements in 15A NCAC 028 .0224, • Section D: 7010-based flow limits have been set for PNA waters per regulations in 15A NCAC 02B. • Section D: TPH monitoring requirements have been added to wastewater monitoring when stormwater runoff from VMA areas commingles with wastewater. • Section D: This wording in the permit has been removed: "For facilities which route stormwater and all process wastewater to a recycle system, no analytical monitoring of process wastewater discharges is required if that recycle system discharges as a result of a 10-year 24-hour (or larger) precipitation event." • Part V • Section A: For existing facilities previously permitted and applying for renewal under this General Permit: New elements of the Stormwater Pollution Prevention Plan for this permit renewal shall be developed and implemented within 6 months of the effective date of the initial Certificate of Coverage issued pursuant to this General Permit and updated thereafter on an annual basis. Your coverage under the General Permit is transferable only through the specific action of DWQ. This permit does not affect the legal requirements to obtain other permits which may be required by DENR, nor does it relieve the permittee from responsibility for compliance with any other applicable federal, state, or local law, rule, standard, ordinance, order, judgment, or decree. Box 399 Kinston, NC 28502 Subject: Notice of Violation V.'O A 3,X1:0,- V- r, I CON L m t+ Stormwater Compliance Inspection Concrete Services of Rocky Mount Tarboro Ready Mix Plant NPDES Stormwater Permit NCG 140007 Rocky Mount, NC Edgecombe County Dear Ms. Huggins: On December 16, 2002 Mr. Myrl Nisely of the Raleigh Regional office of Division of Water Quality made a compliance inspection at the Tarboro facility. The help provided by Mr. Doug Cotton was appreciated. The results are as follows: As at the Rocky Mount plant, the SP3 manual contained good templates for all of the required elements. As at the Rocky Mount plant, all but one of the sections where data should have stored (thus showing implementation of the stormwater program) were empty except for inspection and draining of containment areas. Their omission warrants NOV status for this inspection. Specifically missing were signed monthly housekeeping inspections (Part II(A)(4), semi-annual inspections of the stormwater system(Part II(A)(8) , employee training records (Part II(A)(5), visual observations taken during sampling events Part II(F), and analytical data with chains -of -custody (Part II(C). Analytical data for 2002 was faxed from a corporate office during the inspection. Please add the pH readings taken at the time samples were collected in 2002. If analytical data exists for prior years of this permit, please see that these are placed into the site manual. A NOV is being issued because all of the above records should be stored in a site's manual as they are generated. After the inspection, documents submitted to the State in January cover many of the points above. Records in the SP3 manual should actually go back for the last five years. 2. The Division is asking for more detail on the maps required in Partll(1)(a) and (c). The USGS map (paragraph (a)) needs to be zoomed in enough to allow lines to be added showing each discharge point and where it goes until reaching either a water of the State or a significant tributary. In this case, please show the on -site truck washup path and manmade channel to where it goes under the highway. The name of the eventual receiving stream is to be clearly indicated, as well as latitude and longitude of the discharge location. The sit_ e,man (paragraph (c)) must show stormwater structures, including the path water takes from the on -site truck washup, the manmade channel around the plant to where it goes under the highway, and the unused concrete disposal area where drum washing occurs. At this plant drum washouts simply run onto the ground (more on this in Point 4. below). With a series of arrows on the map, flows of rainwater are to be shown for areas of different topography, with the boundary of each area indicated and the percent impervious surface specified. If the area used for concrete disposal continues to expand each year, NCDENR N. C. Regarding the absence of training records, you report that annual training is given at Kinston all at one time for all locations. The records you submitted documented several topics that were covered, but stormwater training was not among them. After a telephone conversation on 1I2/03 between Mr. Nisely and you, the Division asks that the Plant Manager for each location be responsible for highlighting in color the names of the employees at his site on a copy of the signature sheet, in lieu of individual location signature sheets. (Submitted examples show this will work well.) Training provided'at other times at individual plants should also be documented whenever a stormwater topic is included, as they hopefully would be from time to time. 4. Drum washes are discharged onto the ground behind the hardened unused concrete accumulation. Surface disposal and excavated pits for this high pH water are potential conduits to groundwater. Please be aware that Groundwater Standards apply, and contamination needs to be avoided. 5. Stormwater either runs onto the ground as sheet flow or makes its way to the constructed channel around the plant. The channel is several hundred feet long and well vegetated, helping insure that solids and pH are attenuated by the time the water reaches a tributary. b. Secondary containment structures for fuels and for admixture chemicals were in good condition and the drain handles were locked. Housekeeping was good; the site was free of trash.
