In accordance with your application for a discharge permit received on November 7,
2013, we are forwarding herewith the subject certificate of coverage (COC) to discharge
under the subject state — NPDES general permit. This permit is issued pursuant to the
requirements of North Carolina General Statute 143-215.1 and the Memorandum of
Agreement between North Carolina and the US Environmental Protection Agency dated
October 15, 2007 (or as subsequently amended).
Please take notice that this certificate of coverage is not transferable except upon
approval of the Division of Energy, Mineral, and Land Resources. The Division of Energy,
Mineral, and Land Resources may require modification or revocation and reissuance of the
certificate of coverage.
This permit does not affect the legal requirements to obtain other permits which may
be required by the Division of Energy, Mineral, and Land Resources, the Coastal Area
Management Act, or any other federal or local government permit that may be required.
In addition to your application for coverage under NCG 130000, we also received
your request to rescind coverage under NCG 120000. In accordance with your request, your
coverage under Certificate of Coverage NCG 120021 has been rescinded effectively
immediately.

Dear Sir/Madam:
Smith Gardner, Inc. (S+G) is pleased to submit this Notice of intent (NOI) package for the Feltonsville
Landfill facility located in Wake County, NC. The facility is currently permitted to discharge water from
Landfill related activities under Certificate of Coverage (CDC) Number NCG120021, General Permit
Number NCG120000. As all landfilling activities have ceased, and the landfill has been deemed closed
by the Division of Waste Management, we are asking that the Division rescinds coverage under this
permit (NCG120000).
The only remaining activity on site that requires an NPDES permit is a Multi-MateriaLs Recycling
Facility. As such, we are submitting an N01 for coverage under the General Permit #NCG130000
(Non -Metal Waste and Scrap). Please find the attached NO[ package for coverage under this permit.
Through discussions with the Stormwater Permitting Program, we are not submitting a separate
termination of coverage request for NCG120021 LandflLL COC. This cover letter and the provided
statement included on the N01 form are adequate.

I have discussed this matter with Chris Pullinger, and he has reviewed each of the requests. He reports no problems
with issuance of the COC's, and concurs with rescinding the landfill permits. I recently visited the old Feltonsville Landfill
site on a tour and can confirm that it appears to be complete and stable. Chris also noted no apparent potential wetland
impacts. I can support the actions you have recommended. If there are further questions, please advise and feel free to
communicate directly with Chris.

Hi Dave,
We've received NOls for three existing facilities in your region:
NCG130069 — North Wake Landfill Recycling Facility in Wake County. This site discharges stormwater at one point to an
unnamed tributary to Neuse River (Class WS-IV;NSW,CA). This facility is also asking to rescind their landfill Permit
NCG120020 since the landfill activities have ceased. DWM has deemed the landfill closed. The NO1 is attached.

NCG130070— Feltonsville Landfill Recycling Facility in Wake County. This site discharges stormwater at two points to an
unnamed tributary to Little Branch (Class C). This facility is also asking to rescind their landfill Permit NCG120021 since
the landfill activities have ceased. DWM has deemed the landfill closed. The NO1 is attached.
NCG030643 — Olympic Steel Company in Chatham County. The site discharges stormwater at two points to Blood Run
Creek (Class C) as well as to the Siler City storm sewer system. The NO1 is attached.
Please let me know if the Raleigh Regional Office has any concerns about issuing these three COCs and rescinding the
landfill permits for the two Wake County recycling facilities. Please also let me know of any potential impacts to
wetlands. If we don't receive any objections, we'll issue the COCs in 30 days.
