Hi Bethany,
After our meeting this morning I queried BIMS to see how many "Expired" COCs/permits MRO had. In the past
we have tried to get the expired COCs resolved but it appears new COCs have surfaced for our region.
A few COCs (NCG090002, NCG180121, and NCG180209) we thought had been resolved but they are still
showing as "Expired" when pulled from BIMS. I have attached three documents showing that MRO had asked
permitting to make the COCs "Inactive" since the permittee was no longer at the site and we could not contact
them. The NCG180121 date should be January 2011 not January 2010 as per inspection in BIMS.
Can you let me know who would now handle this in permitting and whether you need the supporting
documentation that we had provided with the Memos.

NCG180209 expired in 2004, so as long as we never received a renewal, you can change ti-le status to inactive.
NCG090002 expired in 2002, so as long as we never received a renewal, you can change the status to inactive.
NCG180121 expired in 2009, so as long as we never received a renewal, you can change the status to inactive.
One question though (which we need to ask Charles about again) is when we inactivate a permit, do we also have to
update the overdue fees in BIMS? I can't remember if we asked that question.

MRO inspected the Regency Leather property on January 6, 2011, as noted in the attached
inspection report. The inspection and associated investigation found that Regency Leather is no longer
conducting furniture manufacturing at the site. The property is not owned by Regency Leather nor any
person associated with Regency Leather. The property has been sold several times since the issuance
of the general permit.
MRO is therefore requesting that Stormwater Certificate of Coverage NCG180209 be made
"Inactive" in RIMS to remove it from the list of facilities needing inspections and remove it from the
associated EPA'permitting and compliance databases available online to the public. .

MRO inspected the Premium Coatings, Inc. property on March 10, 2011, as noted in the
attached inspection report. The inspection and associated investigation found that premium Coatings,
Inc. is no longer manufacturing at the site and the Certificate of Coverage expired in 2002. The
property is currently occupied by Anmar, LLC although the Catawba County GIS system shows
Premium Coatings, Inc. as the property owner. Premium Coatings, Inc. is not a valid business entity as
noted in the attached Secretary of State filings and no contact could be made with Premium Coatings,
Inc. as noted in the attached inspection report.
MRO is therefore requesting that Stormwater Certificate of Coverage NCG090002 be made
"Inactive" in BIMS to remove it from the list of facilities needing inspections.

Inspection Summary:
A site visit was conducted as part of MRO's industrial stormwater inspection program. The Certificate of Coverage for
Premium Coatings expired in 2002_
From the Catawba County GIS website it was determined that the property (Parcel # 373218219162) is still owned by
Premium Coatings, Inc. with a street address of 2200 Silas Creek Parkway, Suite 78, Winston-Salem, NC 27103.
However, the Secretary of State's website notes that Premium Coatings, Inc. business license was suspended since the
company has failed to submit annual reports for the last nine years. A Google search of the companies located at the
Silas Creek Parkway address did not list Premium Coatings, Inc. The contact number for the permit was still in effect
although it was not answered and there was no answering machine linked to it.
The site visit on March 10, 2011, confirmed that Premium Coatings, Inc. is not doing business at the 214 Simpson Street
address in Conover, NC (Catawba County GIS lists the address as 432 Simpson St). The building is occupied by Anmar,
LLC and the lone employee within the building verified that Premium Coatings, Inc. is not based at the facility.
Therefore NCG090002 should be made "Inactive" in RIMS.

NOTICE OF GROUNDS FOR ADMINISTRATIVE DISSOLUTION OF.- Premium Coatings, Inc.
Recently, this office conducted an internal review of the annual reports filed for all business corporations
in North Carolina. Unfortunately, the review indicates that the above referenced business corporation is
not in compliance with the North Carolina Business Corporation Act and is delinquent in delivering nine
(9) annual report(s).
The Secretary of State is authorized by N.C.G.S. 55-14-21 to administratively dissolve a business
corporation if one or more grounds for dissolution exist. Please note that while the dissolution process has
not vet been concluded, this Notice is the first step of the dissolution process. In order to prevent

fifteenth day of the fourth month following the fiscal year end date.

Please be advised that, at the direction of the North Carolina Department of Revenue, the entity named above is hereby
suspended upon the records of the Department of the Secretary of State due to the entity's failure to comply with the
requirements of the Department of Revenue pursuant to N.C.G,S. §105-230(a).
In order to be reinstated with the Department of the Secretary of State, you must contact the North Carolina Department

desired to commence the discharge of wastes or the date on which an existing permit expires, as the case may be".
Any permittee that has not requested renewal at least 180 days prior to expiration or permittee that does not have a
permit after the expiration and has not requested renewal at least 180 days prior to expiration, will be subjected to
enforcement procedures as provided in NCGS §143-215.6 and 33 USC 1251 et. seq.
In order to prevent continued, escalated action, including the assessment of civil penalties you must submit a
completed permit coverage renewal application to the attention of the "Stormwater and General Permits Unit" at
the letterhead address within ten (10) days of your receipt of this letter (renewal application enclosed). If the
subject discharge has been terminated, please complete the enclosed rescission request form. Mailing instructions
are listed on the bottom of the form. You will be notified when the rescission process has been completed.

In response to your renewal application for continued coverage under the subject permit, the Division
of Water Quality (DWQ) is forwarding herewith the reissued stormwater general permit. This permit
is reissued pursuant to the requirements of North Carolina General Statute 143-215 .1 and the
Memorandum of Agreement between North Carolina and the US Environmental Protection agency
dated December 6, 1983.
The following information is included with your permit package:
■ A copy of the stormwater general permit.
n A Stormwater Pollution Prevention Plan Certification Form. This form certifies that you have
developed and implemented the Stormwater Pollution Prevention Plan (SPPP) required in your
permit. This form must be completed and returned to the Division within 30 days of receipt of
this letter. DO NOT send the SPPP with the signed form.
■ Five copies of Analytical Monitoring forms.
■ Five copies of Qualitative Monitoring forms.
■ A copy of a Technical Bulletin on the stormwater program with outlines program components
and addresses frequently asked questions.
m A corrected Certificate of Coverage if you indicated a name or address change on the Renewal
Form returned to the Division.
Your certificate of coverage is not transferable except after notice to DWQ. The Division of Water
Quality may require modification or revocation and reissuance of the certificate of coverage.
This permit does not affect the legal requirements to obtain other permits which may be required by
DWQ or permits required by the Division of Land Resources, Division of Air Quality, Coastal Area
Management Act or any other Federal or Local governmental permits that may be required.

This certificate of coverage shall become effective May 21, 1993.
This Certificate of Coverage shall remain in effect for the duration of the General Permit.
