'Phis certificate of coverage shall become effective October 4, 2013.
This Certificate of Coverage shall remain in effect for the duration of the General Permit.

In accordance with your application for a discharge permit received on September 6,
2013, we are forwarding herewith the subject certificate of coverage (COC) to discharge
under the subject state — NPDES general permit. This permit is issued pursuant to the
requirements of North Carolina General Statute 143-215.1 and the Memorandum of
Agreement between North Carolina and the US Environmental Protection Agency dated
October 15, 2007 (or as subsequently amended).

Please take notice that this certificate of coverage is not transferable except upon
approval of the Division of Energy, Mineral, and Land Resources. The Division of Energy,
Mineral, and Land Resources may require modification or revocation and reissuance of the
certificate of coverage. This permit does not affect the legal requirements to obtain other
permits which may be required by the Division of Energy, Mineral, and Land Resources, the
Coastal Area Management Act, or any other federal or local government permit that may be
required.
We noted that wash bay rinsate is mentioned in the NOI and cover letter from your
consultant. Please be advised that this permit does not authorize the discharge of wash bay
rinsate. Discharges of wastewater, such as wash bay rinsate, may need coverage under an
NPDES wastewater permit.
Please notify this office in writing when the facility is assigned its permanent street
address so that we can update our records.

Brooks Engineering Associates (BEA), on behalf of Houston Transportation, Inc., is
providing the enclosed Notice of Intent for permitting inclusion under NPDES General
Permit NCG 080000.
Houston Transportation is a trucking company currently based in Spruce Pine, NC. They
are planning on moving to a new facility to be located on Ridge Road in Mitchell County.
The discharge to the stormwater system is only for the wash bay rinsate, which will be
routed through an oillgrit separator. Enclosed is the N.0.I along with the $100 application
fee check, site diagram, and location map.
