In accordance with your application for a discharge permit received on September 28, 2011, we
are forwarding herewith the subject certificate of coverage (COC) to discharge under the subject state --
NPDES general permit. This permit is issued pursuant to the requirements of North Carolina General
Statute 143-215.1 and the Memorandum of Agreement between North Carolina and the US
Environmental Protection Agency dated October 15, 2007 (or as subsequently amended).
If any parts, measurement frequencies, or sampling requirements contained in this permit are
unacceptable to you, you have the right to request an individual permit by submitting an individual -permit
application. Unless such demand is made, this certificate of coverage shall be final and binding.
Please take notice that this certificate of coverage is not transferable except after notice to the
Division of Water Quality. The Division of Water Quality may require modification or revocation and
reissuance of the certificate of coverage. This permit does not affect the legal requirements to obtain
other permits which may be required by the Division of Water Quality or permits required by the Division
of Land Resources, Coastal Area Management Act or any other federal or local governmental permit that
may be required.

In compliance with the provision of North Carolina General Statute 143-215.1, other lawful standards and
regulations promulgated and adopted by the North Carolina Environmental Management Commission, and the
Federal Water Pollution Control Act, as amended,

to receiving waters designated as Unnamed tributary to Quankey Creek, a Class C creek, Roanoke River Basin; in
accordance with the effluent limitations, monitoring requirements, and other conditions set forth in Parts I, 11, 111, IV,
V and VI of General Permit No. NCG060000, as attached.
This Certificate of Coverage shall become effective March 9, 2012.
This Certificate of Coverage shall remain in effect for the duration of the General Permit.

Attached is a completed form concerning the Stormwater run-off permit application that
you had advised that Reser's Fine Foods, Inc. needed to submit. I have included a check
for $100.00.
This was in reaction to an observation made concerning some food waste run-off coming
from waste compactors and open top waste containers during our seasonal peak period of
production. As you know,. we have been working with our waste company to insure their
equipment is kept in good shape. Additionally, when we spoke, we discussed a need for
looking at ways to improve our storage area for waste containers.
We are reviewing how we can improve that area of our facilities and are working to get a
plan approved for the back dock area that would improve the containment of food
product waste. Additionally, we are trying to make sure we do the job to completion, so
it can all be done at once. It is very involved with the addition of'conerete and improved
drainage back to the drain below the compactors.
Presently, we are using improved containment by rinsing the trash containers off with a
hose prior to being removed, while in the compactor retention area. Additionally, open
tops are not being used for any waste that might have any wet waste in it. As you know,
the issue with the open tops was a temporary need and is not what we normally do. This
situation was further aggravated by a rain storm during that period.
We plan to have the improved waste area plan approved and start work by the end of
October 2011.
