Enclosed is a copy of the Compliance Evaluation Inspection (CEI) report for the
inspection conducted at the subject facility on August 20, 2012, by Ms. Barbara Sifford of this
Office. The site drains to City of Albemarle Stormwater Collection System which drains into
Little Long Creek which is in the Yadkin Pee Dee River Basin. The site visit and file review
revealed that the subject facility is covered by NPDES Stormwater General Permit NCG030000.
Your assistance and that of Mr. Randy DeVore was greatly appreciated during the inspection.
Accordingly, the following observations were noted during the inspection:
1 Stormwater Pollution Prevention Plan (SPPP) NO
A stormwater pollution prevention plan was developed several years ago, the plan should be
updated to reflect current operations and personnel. An annual review of the plan is required and
this needs to be updated.
2) Qualitative Monitorine NO
Qualitative monitoring has not been recorded and evaluated for the outfalls.
3) Analytical Monitoring NO
No analytical monitoring has been performed on the discharge.

4) Other Observations
A review was made of the site map and corrections were made for the drainage areas and
the outfall sampling location was determined to capture the outside storage of chemicals and
before any contribution from off site or employee parking lot flow would contaminate the
sample.
This report is being issued as a Notice of Violation (NOV) due to the permittee's failure
to comply with the Qualitative Monitoring and Analytical Monitoring Sections of the general
permit NCG030000 issued to Contech US, LLC in November 2007 and expires in October
2012 under Certificate of Coverage number NCG030059 as noted in the attached report.
Pursuant tot G.S. 143-215.6A, a civil penalty of not more than twenty-five thousand dollars
($25,000) per violation, per day, may be assessed against any person who violates or fails to
act in accordance with the terms, conditions, or requirements of any permit issued pursuant to
G. S. 143-215.1.
It is requested that a written response be submitted to this Office by October 26, 2012
addressing the above noted violations in the attached report. In responding to the violations
please address your response to the attention of Ms. Marcia Allocco. The report should be

pursuant to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement
between the state of North Carolina and the U.S. Environmental Protection Agency, dated December 6, 1983.

Your facility is currently covered for stormwatcr discharge under General Permit NCG030000, This permit expires
on August 31, 2002. The Division staff is currently in the process of rewriting this permit and is scheduled to have
the permit reissued by late summer of 2002. Once the permit is reissued, your facility would he eligible for
continued coverage under the reissued permit.
In order to assure your continued coverage under the general permit, you must apply to the Division of Water
Quality (DWQ) for renewal of your permit coverage. To make this renewal process easier, we are informing you in
advance that your permit will be expiring. Enclosed you will find a General Permit Coverage Renewal
Application Form. Tile application must be completed and returned by March 4, 2002 in order to assure continued
coverage under the general permit.
Failure to request renewal within this time period may result in a civil assessment of at least $250.00. Larger
penalties nlay be assessed depending on the delinquency of the request. Discharge of stornwater from your facility
without coverage under a valid storrnwater NPDES permit would constitute a violation of NCGS 143-215.1 and
could result in assessments oE' civil penalties of up to $10,000 per day.
Please note that recent federal legislation has extended the "no exposure exclusion" to all operators ol' industrial
facilities in any of the 1 I categories of "storm water discharges associated with industrial activity," (except
construction activities). If you feel your facility can certify a condition of "no exposure", i.e. the facilty industrial
materials and operations are not exposed to stormwatcr, you can apply For the no exposure exclusion. For additional
