Dear Mr. Roark:
On May 25, 2016 Glen White and Sue White of the North Carolina Department of
Environmental Quality; Department of Minerals and Land Resources met with Johnny Hampton
at the recycle yard located at 5251 US Hwy 431 S, Boone; to conduct a compliance inspection
per the Industrial Stormwater Permit requirements. Inspection consist of review of the
Stormwater Pollution Prevention Plan, training and monitoring records and observation of the
general condition of the site, outfalls and any possible exposures. His assistance with the
inspection was greatly appreciated.
Permit:
This facility holds General Stormwater Permit NCG020744 to discharge stormwater from
industrial activity associated with Mine Dewatering and Process Wastwater under the
National Pollutant Discharge Elimination System (NPDES). The permit became effective
January 01, 2010 and expired December 31, 2014. A copy of the current permit is required to be
maintained with the Stormwater Pollution Prevention Plan (SPPP) documents.
Records/Reports:
This facility was required to develop and maintain a Stormwater Pollution Prevention Plan
(SPPP) in accordance with Part II, Section A of the permit. The SPPP along with monitoring
and training records need to be updated annually. All required components of the SPPP
including the siteplan, training records, spill and monitoring records need to be maintained in one
3-ring binder that is within easy reach for maintenance, review and inspections by both your staff
and by the inspector. Please provide one complete hard copy to be kept in the scale house office
onsite. All records must remain with the file for a minimum of five (5) years.

Facility Site Review:
This facility is a recycle yard for stone and asphalt materials. Mr. Johnny Hampton currently
runs the facility but will retire following this summer. He recently sold the operation to
Maymead Materials and agreed to run the facility throughout the year. The site has two
sediment basins with baffles. They appear to be maintained. There is a scale house with a truck
scale and there is crushing and sizing equipment for crushing rock and asphalt.
Effluent Receiving Waters:
Effluent from this facility drains to receiving waters of Thaxon Creek, Class C waters of the New
River Basin.
Self -Monitoring Pro ram:
Only qualitative monitoring is required under current permit requirements and current operating
procedures. Monitoring has been performed as requited. Monitoring Records need to be
maintained at the site. Keep records in the SPPP and maintain for a minimum of (5) five years.

i received a request from Sheila Holman of DAQ asking if we could check our sediment program and stormwater program
records for any outstanding/unresolved violations cited at any Maymead Materials facility in NC (this may affect the
issuance of pending DAQ air quality permits). From DAQ records, there are 6 facilities in ARO, 3 in the MRO, and 5 in the
WSRO.
Can you search your NOV records to determine if there are any open/unresolved/outstanding violations at any of these
facilities? If so, please provide the project number; project name, county, type of NOV (SPCA and/or SW), NOV issue date
and deadline date, and date of last non-compliance inspection. Can you please send me this information (or confirm
there are no outstanding violations for the facilities in your region) by COB today or at least by Noon on Monday?

I found three facilities, all in Watauga. One of them was inspected in October 2015 by myself and Sue White. It was in
violation and I haven't been back there yet to see if they've improved. Pooriy maintained facility that had never
maintained its stormwater permit and didn't seem concerned. The other two facilities are due to be inspected so 1 need
to get those inspections planned.
