In accordance with your application for a discharge permit received on March 21, 2007, we are
forwarding herewith the subject certificate of coverage to discharge under the subject state — NPDES
general permit. This permit is issued pursuant to the requirements of North Carolina General Statute 143 '
215.1 and the Memorandum of Agreement between North Carolina and the US Environmental Protection
Agency dated May 9, 1994 (or as subsequently amended).
Please take notice that this certificate of coverage is not transferable except after notice to the
Division of Water Quality. The Division of Water Quality may require modification or revocation and
reissuance of the certificate of coverage. Please also note the requirement to have a Stormwater Pollution
Prevention Plan and to comply with the cut off concentrations listed in the permit.
This permit does not affect the legal requirements to obtain other permits which may be required
by the Division of Water Quality or permits required by the Division- of Land Resources, Coastal Area
Management Act or any other federal or local governmental permit that may be required.

NCG020000 as attached.
This certificate of coverage shall become effective April 13, 2007.
This Certificate of Coverage shall remain in effect for the duration of the General Permit.

Dear Mr. Cecile:
The Division of Land Resources has informed our office that you have applied for a transfer of the
mining permit from Mr. Rodney and Paul Henkel to yourself. You are required by North Carolina
General Statute 143-215.1 (a)(1) to obtain a General Permit (NCG020000) to discharge stormwater
under the National Pollutant Discharge Elimination System (NPDES) in accordance with Title 15A'
North Carolina Administrative Code 2B .0100,
The General Permit requires that you: implement a stormwater plan, perform and document
monitoring of the outfall discharge(s), and comply with record keeping requirements. (See attached
Technical Bulletin) During annual inspections, records must be made available when requested by
staff from the Division of Water Quality.
Please respond to this office. in writine. within 15 business days of receipt of this letter. The
response should include the date you sent the Notice of Intent to the Stormwater and General Permits
Unit located in Raleigh. (Application enclosed) Upon receiving the permit, you should read and
follow all of its terms and conditions. If you have any questions or need assistance, please contact
George Smith at (336) 771-5000 so that he can meet you on site.

Dear Mr. Cecile:

Your request to have the above referenced mining permit, issued to Rodney Henkel
and Paul Henkel, transferred in its entirety to your company and modified to change the mine
name from Mountain Stone Quarry to Glendale Springs Quarry has been approved. As we
have received the required surety in an amount sufficient to cover the present operation, I am
enclosing the transferred mining permit. The permit number and expiration date shall remain
the same as before the transfer.
Please review the permit carefully and notify this office of any objections or questions
you may have concerning the terms of the permit. It is your responsibility to fully understand
and comply with the conditions of your mining permit. I would like to draw your particular
attention to the following conditions where minor additions or changes were made: Operating
Condition Nos. 3C, 48, 8A through J and 14 and Reciamation Condition No. 3.
As a reminder, your permitted acreage at this site is 9 acres and the amount of land you
are approved to disturb is 3.25 acres.
As we have received the $42,900.00 Assignment of savings Account issued by High
Country Bank to cover this site and Mining Permit No. 95-03 (Bamboo Road Quarry), I am
returning the $29,100.00 Assignment of Savings Account issued by High Country Bank
previously held for Mining Permit No. 95-03 to you for your disposal.
