In response to your renewal application for continued coverage under Stormwater General Permit
NCG020702 August 7, 2012, the Division of Water Quality (DWQ) is forwarding herewith the reissued
Certificate of Coverage and stormwater General Permit. This permit is reissued pursuant to the
requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement between
the state of North Carolina and the U.S. Environmental Protection Agency, dated October 15, 2007 (or as
subsequently amended.)
The following information is included with your pert -nit package:
• A new Certificate of Coverage
• A copy of the stormwater General Permit
• A copy of a Technical Bulletin for the General Permit
• Copies of the Discharge Monitoring Report (DMR) Form
• Copies of the Annual Discharge Monitoring Report Form (if applicable)
• Copies of the Qualitative. Monitoring Report Form
The General Permit authorizes discharges of stormwater only, and it specifies your obligations with
respect to stormwater discharge controls, management', monitoring, and record keeping. Please review
the new permit to familiarize yourself with all changes in the reissued permit. Significant changes to the
General Permit are outlined in the attached Technical Bulletin.
Your coverage under the General Permit is transferable only through the specific action of DWQ.
This permit does not affect the legal requirements to obtain other permits which may be required by
DENR, nor does it relieve the permittee'from responsibility for compliance with any other applicable
federal, state, or local law, rule, standard, ordinance, order, judgment, or decree.

On August 14, 2012 a Compliance Evaluation Inspection was performed by George Smith, of this office. Ms.
Patricia Jordan, President, Mr. Larry Shumate, and Mr. Mark McNeil were present for the inspection. Tim
Latham with the Land Quality Section was present for a mine inspection.
Permit
This General Permit became effective January 1, 2010 and expires on December 31, 2014. Please read the
permit and insure that you are compliant with all terms and conditions.
Part III, Section A - Stormwater Pollution Prevention Plan (SPPP)
The SPPP is on file and being upgraded. Please insure that all nine (9) requirements of this plan are addressed
and documented prior to the next inspection. Please refer to the simplified SPPP model sent to your email
(tricopavingaaskybest.com) on August M, 2012. A more thorough model to follow.
Facility Site Review
There is no Vehicle Maintenance Area. There is no Mine dewatering, Stone Washing, or Recycle Wastewater
discharges. There is no Closed Loop Recycle system.
Laboratory
Analytical tests, such as, Settleable Solids, Total Suspended Solids (TSS), Turbidity, and pH were all performed
using approved analytical methods by a certified lab.

Part III, Section B — Stormwater Analytical Monitoring
Analytical tests, such as, Settleable Solids, Total Suspended Solids, and Turbidity are performed semi-annually
at Outfall 001 by using approved analytical methods by a certified lab/person. These tests are only required
semi-annually.
Rainfall is measured and documented in a separate log.
The current permit, which became effective on January 1, 2010, requires that each outfall be. sampled twice per
year as shown in Table 2 (January — June and July — December) for the following:

Effluent/Receiving Waters
The stormwater discharges to Little Buffalo Creek, Class C; Trout waters of the ,State. The waters are not
