For coverage under NPDES General Permit NCG020000, the Division of Energy, Mineral and Land
Resources (DEMLR) is forwarding herewith the reissued Certificate of Coverage and
Stormwater/Wastewater General Permit. This permit is reissued pursuant to the requirements of North
Carolina General Statute 143-215.1 and the Memorandum of Agreement between the state of North
Carolina and the U.S. Environmental Protection Agency, dated October 15, 2007 (or as subsequently
amended.) The following information is included with your permit package:

• Technical Bulletin for the General Permit
• Discharge Monitoring Report (DMR) Forms
• Annual Discharge Monitoring Report Forms
• Qualitative Monitoring Report Forms
The General Permit authorizes discharges of stormwater and/or wastewater (as applicable to your site),
and it specifies your obligations with respect to discharge controls, management, monitoring and record
keeping. Please review the new permit to familiarize yourself with all changes in the reissued permit.
Significant changes to the General Permit are outlined in the'1'echnical Bulletin.
Your coverage under the General Permit is transferable only through the specific action of DEMLR, This
permit does not affect the legal requirements to obtain other permits which may be required by the N.C.
Department of Environmental Quality, nor does it relieve the pertmittee from responsibility for
compliance with any other applicable federal, state, or local law, rule, standard, ordinance, order,
judgment or decree.

In accordance with your application for a discharge permit received on
September 7, 2006, and the issuance of your Division of Land Resources mining permit
on June 18, 2009, we are forwarding herewith the subject certificate of coverage to
discharge under the subject state — NPDES general permit. This permit is issued
pursuant to the requirements of North Carolina General Statute 143-215.1 and the
Memorandum of Agreement between North Carolina and the US Environmental
Protection Agency dated October 15, 2007 (or as subsequently amended).
This general permit allows the discharge of stormwater from your facility. In your
application, you specified that only stormwater would be.discharged from this facility. If
separate point -source discharges of wastewater occur (i.e., mine dewatering water or
other process wastewater), you will be required to seek a modification to� this permit.
Any modification to add a wastewater discharge forth is facility would have to be
reviewed and issued prior to that discharge occurring at your facility.
This permit does not affect the legal requirements to obtain other permits which
may be required by the Division of Water Quality or permits required by the Division of
Land Resources, Coastal Area Management Act or any other federal, state or local
governmental permit that may be required.

Other notes:
This mine is difficult to locate. The application says that the address is 1860 Concord -Davidson
Highway. Google Maps only knows this as Davidson -Concord Highway (also known as Highway 73),
and cannot find the address 1860. The application also says that it is at the intersection of Poplar Tent
and Highway 73 (see Map 3), and the map provided shows "Columbus Chapel", which is near Highway
73 and Shiloh Church Rd (see Map 3). However, the stretch of Shiloh Church Rd that intersects with
Highway 73 is named Stanley McElrath Rd (see Map 2). The mine is near this intersection, and between
two side roads, Kingsway Drive and Vickers Rd (see Map 1).
