Inspection Summary:
The purpose of this inspection was to follow-up on the May 20, 2014 inspection. Since the May 20, 2014 inspection, the
company has hired a contractor to prepare a SPPP, and staff training has occurred. Areas of improvement needed for the
plan include labeling the outfall locations and drainage areas on the plans, documenting oil usage.
Other notes: Maintenance area for changing equipment buckets needs addressed. Sampling must occur within the
required timeframes listed on the permit. Sampling for the storwater discharge is to occur semi-annually [description begins
on page 4 of the permit], while sampling for the mine dewatering wastewater is to occur quarterly [description begins on
page 11 of the permit].
The general permit for this facility expires December 31, 2014. On-line request for renewal of this permit was verified and is
under review.

Inspection Summary:
This was the first compliance inspection for the industrial stormwater permit. The facility contact was not familiar with
industrial permit requirements. A copy of the permit will be provided with the inspection letter, and a follow-up inspection
to determine whether the permit is being implemented will be conducted after September 2, 2014.

are forwarding herewith the.subject certificate of coverage to discharge under the subject state —
NPDES general permit. This permit is issued pursuant to the requirements of North Carolina
General Statute 143-215.1. and the Memorandum of Agreement between -North Carolina and the
US Environmental Protection Agency dated May 9, 1994 (or as subsequently amended).
Please take notice that this certificate of coverage -is not transferable except after notice to
the Division of Water Quality. The Division of Water Quality may require modification or,
revocation and reissuance of the certificate of coverage.
