Skvarla, III Secretary On May 20, 2014, personnel of the Land Quality Section performed an inspection of your facility and observed the facility to be inactive. Please find attached the mine inspection report. The mine permit expires July 28, 2014, and if you have not already done so, please contact our Mining Program staff at (919) 707-9220 to discuss permit renewal and mining reclamation requirements. An inspection report regarding industrial stormwater is also attached. On March 3, 2004, the former Division of Water Quality issued correspondence stating the NPDES Permit Application for the referenced facility was incomplete and therefore returned. Therefore, this facility does not have in effect an industrial stormwater discharge permit. No activities resulting in stormwater discharge may occur until you obtain an industrial stormwater permit for this facility. For your convenience, a copy of the application for the industrial stormwater permit is attached. As stated in the Notice of Intent (NOI), a copy of the valid approved mining permit for the facility must be attached to the NOI in order for the stormwater permit application to be considered complete. To date, we have not received a copy of the mining permit for this facility. Therefore, the Division of Water Quality is returning the application as incomplete. This application may be resubmitted with all applicable attachments and fees for reconsideration. Dear Mr. Watts: A review of your mining permit application by the Mooresville Regional Office/Division of Water Quality staff revealed that no storm water permit application was submitted for the subject site. Mr. Samar bou-Ghazale of my staff contacted your Engineer Mr. Carroll Williams who confirmed that a storm water application was not submitted with the application package. A storm water permit (NCG020000) is required for the subject site. In order to comply with NCGS 143-215.1, please fill out the enclosed application and return same to the address shown on the application.
