Inspection Summary:
Mr. Heath (HR Manager/facility contact) was unable to locate a Stormwater pollution prevention plan during this multi -media
inspection, though the facility was conducting its qualitative and analytical monitoring. Mr. Heath contacted Mr. David
Vernon (former facility contact)in regards to this issue. Mr. Vernon stated that the facility had been granted an exemption,
from DWQ, that allowed them to operate in compliance without having to create an SPPP. Upon returning to the Winston
Salem Regional Office (WSRO) this inspector consulted with Gary Hudson (former DWQ Stormwater Inspector) about this
issue. Mr. Hudson verified that NC Granite Corp. had been granted this exemption as allowed by Part III - Stormwater
Pollution and Prevention Plan section of NC General Permit No. 020000, which states in part; "The SPPP shall be
maintained on site unless exempted from this requirement by the Division" (page 1, paragraph 1), Upon further investigation,
it was discovered that Mr. Hudson had also given written statement to this exemption within the SW comliance inspection
report dated March 19, 2008, Since that time, the Stormwater Compliance Program has been transferred from the Division
of Water Quality to the Division of Energy, Mineral and Land Resources (DEMLR). This inspector consulted, on this
matter, with Mr. Glen White, Environmental Specialist and Stormwater Compliance Inspector of DEMLR. It was decided by
Mr. White that NC Granite Corp. would be required to create and implement a Stormwater Pollution Prevention Plan from
this date forward, in accordance with Part III (pages 1-5)of General Permit #NCG020513. A follow-up inspection will be
conducted, by Mr. Glen White, within the next 9-12 months.

SAMPLING INSTRUCTIONS AND FORM COMPLETION
FAILURE TO PROPERLY CHILL, CHEMICALLY PRESERVE, COLLECT IN PROPER BOTTLE
TYPES, MEET REQUIRED HOLDING TIMES AND NEUTRALIZE CHLORINE IN CHLORINE SEN-
SITIVE SAMPLES MAY RESULT IN SAMPLES BEING REJECTED BY THIS LABORATORY AS
PER NORTH CAROLINA REGULATORY CODE. .
1) Samples not falling within the established guidelines will need to be re -collected. The client will be contacted and informed of the
deviation and asked to collect another set of samples. The client may wish for the laboratory to proceed with the analyses of the current
samples. Any samples analyzed outside of the required guidelines will be "qualified". This means that a note will be included on the
sample result and "Chain of Custody" specifying the deviation. The laboratory is also required to send a letter to the State noting the
deviations.
2) Sample Temperature. Samples for compliance monitoring must be chilled with wet ice to a temperature of 6°C or, less. Freezing is
not permitted. Samples delivered to the lab shortly after collection may not have had enough time to be chilled below VC. In this case,
the temperature at time of collection must be noted in the space provided. The samples will meet the requirements of the regulation if
there is a temperature drop from the time of collection until received in the lab. Regardless, all samples should be packed in wet ice, using
as much ice as will fit in the cooler.
3) Sample Chemical Preservation. Many samples require a chemical preservation, such as Sulfuric Acid or Sodium Hydroxide. The
laboratory will either provide the preservative in the sample bottle, or in the case of 40 ml Volatiles Vials, provide a bottle of Acid with
detailed descriptions on how to collect the sample. Never rinse sample bottles before collecting samples. Any residue or liquid in the
bottle is required for proper chemical preservation, The lab must verify proper chemical preservation upon arrival in the lab and will note
this information in the spaces provided on the front of this form.
4) Chlorine Neutralization. Some samples require that any Total Chlorine Residual be removed at the time of collection. The lab will
provide the proper neutralizing agent in the sample bottle when technically possible. There are some samples (Total Kjeldahl Nitrogen
and Ammonia Nitrogen) where this is not possible due to interferences between the required chemical preservation (Acid) and the
dechlorinating agent. Therefore, these samples must be dechlorinated at the time of collection before being placed in our sample bottles.
.Sodium Thiosulfate is the chemical of choice to neutralize chlorine. It must be added to your sample and then the sample checked for
Total Chlorine before the sample is poured in our bottle. Facilities using chlorine for disinfection should have a means of measuring
Total Chlorine. Non -chlorinated sample sources will not need to be checked. It is also required that you note the "Total Chlorine at
Collection" on the front of this foram for any sample locations applicable. This value would be before any neutralization is performed.
5) Other information required to be completed by the client are:
Collection Date and Collection Time for each sample location • Temperature at Time of Collection
Signature and prL-ited name of person or persons collecting samples • Signature, Date and Time samples were relinquished
Other added sample locations and analyses required
Deletion on the form for any samples which are not needed (example: dry upstream location)
Any other information felt to be pertinent should be included in the "Remarks" section.
CONSIDERATIONS:
Coliform samples have a holding time of 6 hours from the time of collection to time of analysis. Therefore, samples should be collected
as late in the day as possible to allow enough time for transportation, checking in at the lab and analysis.
BOD samples have a 48 hour holding time and are set in the lab Wednesday through Friday.
Other samples, such as Nitrate, Ortho Phosphorus, Settleable Matter, Turbidity, Color and MBAS, also have a 48 hour holding time. The
lab reserves the right to establish required sample collection and delivery dates in order to meet required holding times.
CAUTION
These sample bottles may contain acid or other corrosive and potentially harmful chemicals. Laboratories are required
to add these chemicals for certain analyses in order to comply with EPA preservation requirements. Use extreme care
when opening and handling the bottles. If any chemical should get on your skin or clothes, flush liberally with water and
seek medical attention.

On March 20, 2013, Gary Hudson -of this office met with you to perform a General
Stormwater Permit Inspection at the North Carolina Granite Corp. Mount Airy facility located off
of Highway 103 in Mount Airy. This inspection evaluated the areas as. designated on the
attached inspection form. Observations from each area are addressed below:
1. Permit
North Carolina Granite Corp. holds Certificate of Coverage # NCG020513 for the discharge of
stormwater from the facility. The General Permit became effective January 1, 2010 and expires
December 31, 2014.
2. Records/Reports
All monitoring and laboratory records are maintained as specified by the permit.
3. Facility Site Review
The site appeared very clean and well managed. However, at the time of the inspection it was
noted that the effluent from the wastewater pond was slightly turbid. The upper settling pond
was found to be full of solids, and needed to be cleaned out. It is our understanding that the
pond will be cleaned out within the next two weeks.
4. Effluent/Receiving Waters
Stormwater runoff from the facility reaches the Ararat River, Class "C" waters in. the Yadkin/Pee-
Dee River Basin.

5. Monitorinq Program
Part III, Section B of the permit requires that semi-annual qualitative (visual) monitoring be
performed and documented once in the spring (April - June) and once in the fall (September —
November). A review of the facility files found that the qualitative monitoring has been
conducted as required.
Part 111, Section C of the permit requires that analytical monitoring be performed for Mine
Dewatering Wastewater. A review of the facility files found that the analytical monitoring has
been conducted as required.
The analytical monitoring for on -site vehicle maintenance iisted in Section B.of the permit is
required since the NC Granite Corp. plant uses more than 55 gallons of new motor oil per month
when averaged over the calendar year. A review of the facility, files found that the annual
analytical monitoring of oil & grease has been conducted as required.

On February 6, 2009, Gary Hudson of this office met with you to, perform a General
Stormwater Permit Inspection at the North Carolina Granite Corp. Mount Airy facility located off
of Highway 103 in Mount Airy. This inspection evaluated the areas as designated on the
attached inspection form. Observations from each area are addressed below:
1. Permit
North Carolina Granite Corp. holds Certificate of Coverage # NCG020513 for the discharge of
stormwater from the facility. The General Permit became effective January 19, 2005 and
expires December 31, 2009,
2. Records/Reports
All monitoring and laboratory records are maintained as specified by the permit.
3. Facility Site Review
The site appeared very clean and well managed. During the last inspection upon observing the
Stormwater Discharge Outfall.coming from the L.S.V. plant, it was noticed that this might be an
intermittent conveyance of process wastewater discharge. It was agreed upon at the time of
that inspection that the overflow piping from this process (a closed loop system) be re -piped into
the process wastewater treatment system. It was confirmed during this inspection that the
overflow piping from the L.S.V. plant had been piped to the wastewater treatment system, and
our office appreciates your cooperation in this matter.
4.Effluent/Receiving Waters
Stormwater runoff from the facility reaches the Ararat River, Class "C" waters in the Yadkin/Pee-
Dee River Basin,
