On January 28, 2015, the Division of Energy, Mineral and Land Resources received your request to rescind
your coverage under Certificate of Coverage Number NCG020340. In accordance with your request,
Certificate of Coverage Number NCG020340 is rescinded effective immediately.
Operating a treatment facility, discharging wastewater or discharging specific types of stormwater to waters
of the State without valid coverage under an NPDES permit is against federal and state laws and could result
in fines. If something changes and your facility would again require stormwater or wastewater discharge
permit coverage, you should notify this office immediately. We will be happy to assist you in assuring the
proper permit coverage.
If the facility is in the process of being sold, you will be performing a public service if you would inform the
new or prospective owners of their potential need for NPDES permit coverage.

Aana Taylor -Smith of the Winston-Salem Regional Office of the NC Division of Water Quality (DWQ) conducted a
compliance evaluation inspection at the subject facility on 18 July 2013. The assistance and cooperation of Mr.
Lindsey Tuttle were greatly appreciated. An inspection checklist is attached for your records and the inspection
findings are summarized below.
The facility is located on the east side of Crutchfield Road, approximately one mile north of the intersection of
Crutchfield Road and NC Highway 29, in Rockingham County, North Carolina. Stormwater related to industrial
activity is discharged to Wolf Island Creek, which is currently classified as Class C waters in the Roanoke River
Basin.
Documentation and Monitoring Review
The Stormwater Pollution Prevention Plan (SPPP) is complete and current. All training, maintenance, and
inspection records were thorough and up-to-date. A Best Management Practice (BMP) plan was available at the
site, as well as complete BMP inspection records. The Erosion and Sediment Control Plan and Mining Permit
issued by the NC Division of Energy, Mineral, and Land Resources (DEMLR) were also available for inspection.
Qualitative and analytical monitoring have been conducted and documented as required. The borrow pit has
one Stormwater Discharge Outfall (SDO) at which qualitative and analytical monitoring are performed as
required after each discharge. All analytical monitoring results were within benchmark values and no problems
were encountered. It was noted that due to the site's characteristics, a discharge is very rare. No vehicle
maintenance or wastewater treatment occurs at this site.
The previous inspection, performed in March 2009, revealed several discrepancies and issues with the SPPP,
BMP plan, and monitoring. Ms. Taylor -Smith was pleased to find that all of those problems had been addressed
and corrected. The City of Reidsville's dedication to compliance is highly appreciated.

Facility Site Review
The facility is clean and well -maintained. The borrow pit is stabilized with vegetation and has,not been used in
over one year. No secondary containment is necessary at this site. A spill kit is available on site, as per the Spill
Prevention and Response Plan (SPRP). A rain gauge was also located on site, as required by the permit, Outfall
001, which is also the site's BMP, was inspected. The SDO was clean and well -maintained. No evidence of
erosion or instability at the SDO was observed.
We appreciate your efforts to comply with the stormwater general permit, as well as the improvements made to
the facility's SPPP and practices since the last inspection. No additional response to this letter is required. Please

1. Ron Boone of the Winston-Salem Regional Office of the NC Division of Water Quality (DWQ)
conducted a compliance evaluation inspection (CEI) at the subject facility on 24 March 2009.
Your assistance and cooperation during the inspection, as well as that of Mr. Lindsey Tuttle, was
greatly appreciated. An inspection checklist is attached for your records and the inspection
findings are summarized below.
2. The facility is located on east side of Crutchfield Road approximately one mile north . of the

3. A BMP plan was not available at the site during the inspection. Inspection records for BMPs are
also not being kept. Please ensure the BMP plan is kept on site at all times and begin inspecting
all BMPs as reflected in your approved Erosion & Sedimentation Control Plan for the site. Please
ensure that all measures are inspected in accordance with the requirements of the permit.
4. Neither the required qualitative monitoring nor the -analytical monitoring has been accomplished as
required. Please sample as soon as possible and notify Mr. Boone when it has been completed
and provide him a copy of the sampling results. Please note that the general permit is currently
being rewritten and significant changes to the monitoring requirements are expected. If you need
to obtain a copy of the latest NCG020000 general stormwater permit, you may do so at any time

5. The site was very clean and well maintained. Stormwater is conveyed to a sediment trap located
on the northwest corner of the site, which then discharges to the receiving stream. No further
issues were noted at the site.

to correct the deficiencies noted above in paragraphs 3 and 4 and keep Mr. Boone up to date on
